摘要
中风后肩手综合征为中风后偏瘫患者的常见并发症,其多表现为患侧肩、手关节出现疼痛、活动受限、手部浮肿、皮温肤色改变,甚至挛缩畸形。不仅影响患者的生活、工作,同时对患者的心理带来沉重的负担。本病的发病机制复杂,病情易反复,目前西医治疗手段多以康复为主,但效果较单一,配合中医针灸结合治疗疗效更为显著。孙秀萍主任医师认为中风后肩手综合征多以痰瘀阻滞,气血亏少为主要病机,主张头针、体针与雷火灸相结合治疗,针刺治疗补益气血,疏通经络,雷火灸温阳化痰,化瘀通络,两者合用具有补益气血,通络止痛,祛痰行瘀的功效,值得临床推广及应用。
Shoulder hand syndrome after stroke is a common complication in patients with hemiplegia after stroke.It is often manifested as pain,limited mobility,hand swelling,skin temperature and skin color changes,and even contracture deformities in the affected shoulder and hand joints.Not only does it affect the life and work of patients,but it also brings a heavy burden to their psychology.The pathogenesis of this disease is complex,and the disease is easy to recur.At present,western medicine treatment mainly focuses on rehabilitation,but the effect is relatively simple,and the combination of traditional Chinese medicine and acupuncture and moxibustion treatment is more effective.Sun Xiuping,Chief physician,believes that the main pathogenesis of shoulder hand syndrome after stroke is the blockage of phlegm and blood stasis,and the lack of qi and blood.He advocates the combination of scalp acupuncture,body acupuncture and thunder fire moxibustion.Acupuncture treatment is to replenish qi and blood,dredge the meridians,and thunder fire moxibustion is to warm the yang to remove phlegm,and to remove blood stasis and dredge the collaterals.The combination of the two has the effects of supplementing qi and blood,dredging the collaterals to relieve pain,and removing phlegm and blood stasis,which is worth clinical promotion and application.
