摘要
目的探讨分析评估彩色多普勒超声诊断2型糖尿病下肢动脉病变的临床有效率。方法选取我院收治的60例2型糖尿病下肢动脉病变患者,设为观察组,选取同期于我院体检的健康人60例设为对照组,均开展彩色多普勒超声检查,比较两组下肢动脉病变检出情况。结果观察组内膜粗糙占91.7%,斑块占51.7%,闭塞占31.7%,狭窄占61.7%,均明显高于对照组,差异有统计学意义(P<0.05);观察组腘动脉病变发生率为28.3%,胫后动脉为33.3%,股总动脉为26.7%,足背动脉为48.3%,均明显高于对照组,差异有统计学意义(P<0.05);观察组腘动脉、胫后动脉、股动脉及足背动脉的血管内径均比对照组窄,中膜厚度均比对照组厚且血流速度均比对照组慢,差异有统计学意义(P<0.05)。结论彩色多普勒超声诊断糖尿病下肢动脉病变的有效率高,且价格低廉、无创、直观及可重复等,有推广价值,可列为2型糖尿病患者定期检查的常规项目之一,以对糖尿病足等并发症予以有效预测并指导临床制定治疗方案,最终改善患者生活质量。
Objective To evaluate the clinical effectiveness of color Doppler ultrasonography in the diagnosis of lower ex‐tremity arterial disease in type 2 diabetes mellitus.Methods In this study,60 patients with type 2 diabetes lower extremity ar‐terial disease admitted to our hospital were selected as the observation group,and 60 healthy people who underwent physical ex‐amination in our hospital during the same period were selected as the control group.Color Doppler ultrasound examination was carried out to compare the detection of lower extremity arterial disease between the two groups.Results In the observation group,91.7%of endometrial roughness,51.7%of plaque,31.7%of occlusion and 61.7%of stenosis were significantly higher than those in the control group(P<0.05).In the observation group,the incidence of popliteal artery lesions was 28.3%,that of posterior tibial artery was 33.3%,that of common femoral artery was 26.7%,and that of dorsal foot artery was 48.3%,which were significantly higher than those in the control group(P<0.05).The inner diameter of popliteal artery,posterior tibial artery,femo‐ral artery and dorsal foot artery in observation group were narrower than that in control group,and the thickness of middle mem‐brane was thicker and the blood flow velocity was slower than that in control group(P<0.05).Conclusion Color doppler ultra‐sound has high in diagnosing pathological changes of diabetic lower extremity artery,with high efficient,low cost,as well as non‐invasive,intuitive and repeatable traits,which can be classified as one of regular inspection of conventional projects for type 2 diabetes,thereby effectively predicting the diabetic foot complications,guiding the clinical treatment,and eventually improving patients'quality of life.
