北京市40所大型公立医院远程医疗服务现状与对策研究

The status and strategies of telemedicine services in 40 large public hospitals in Beijing

摘要 目的:对北京市各系统医疗机构远程医疗业务现状进行调查,了解远程医疗服务面临的机遇与挑战,为促进远程医疗发展提供参考和依据。方法:以北京市各系统医疗机构为研究对象,采用问卷调查方式收集远程医疗服务开展的相关信息,并进行描述性统计学分析。结果:调查的40所医院开展远程会诊业务占90%,设置远程业务职能占95%,配备工作人员占92.5%,年度投入不足1万元的占67.5%、会诊量不足300例的占72.5%、会诊收入不足10万元的占75%。认为专家精力有限是影响远程发展主要原因的占65%,认为远程模式可以提升基层诊疗能力、降低患者负担、提升医院影响力的占比均在75%以上。结论:对远程医疗服务在提升医院品牌、助力分级诊疗、方便患者就医、减轻患者负担等方面的作用认同度较集中,各医院在远程人才培养、学科宣传、科研等方面投入明显不足,医保政策、信息技术创新应用、服务规范和行业服务标准亟待进一步加强和完善。 Objective:To investigate the current status of telemedicine services in medical institutions across Beijing and understand the opportunities and challenges faced by telemedicine services,providing references and bases for promoting the development of telemedicine.Methods:Taking medical institutions in Beijing as the research subjects,a questionnaire survey was used to collect information related to the provision of telemedicine services,followed by a descriptive statistical analysis.Results:Among the surveyed hospitals,90%offered teleconsultation services,95% had established telemedicine departments,92.5% had designated staff for telemedicine,67.5%invested less than 10,000 yuan annually,72.5% had fewer than 300 consultation cases annually,and 75% earned less than 100,000 yuan from consultations annually.65% believed that the limited availability of specialists was the main factor affecting the development of telemedicine.Over 75% agreed that telemedicine models could enhance primary care capabilities,reduce patient burden,and increase hospital influence.Conclusion:There is a concentrated recognition of the role of telemedicine services in enhancing hospital branding,aiding hierarchical diagnosis and treatment,facilitating patient medical treatment,and alleviating patient burden.However,investments in remote talent training,department promotion,and research are clearly insufficient among hospitals.Health insurance policies,information technology innovations,service regulations,and industry service standards urgently need further strengthening and improvement.
