摘要
目的:分析低分子肝素治疗胎儿生长受限的效果。方法:选取2021年4月—2022年4月惠州市中心人民医院及惠州市第一人民医院收治的胎儿生长受限孕妇70例为研究对象。依据随机数字表法分为对照组和观察组,各35例。对照组行传统治疗,观察组在对照组基础上应用低分子肝素治疗。比较两组胎儿生长发育指标、孕妇宫高、凝血功能、胎盘血流动力学指标、妊娠不良结局及不良反应发生情况。结果:治疗2周后,两组胎儿腹围、头围、股骨长及孕妇宫高增大,观察组大于对照组,差异有统计学意义(P<0.05)。治疗2周后,两组孕妇活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)水平升高,D-二聚体(D-D)及纤维蛋白原(FIB)水平降低,观察组APTT、PT水平高于对照组,D-D、Fib水平低于对照组,差异有统计学意义(P<0.05)。治疗2周后,两组胎儿脐动脉搏动指数、阻力指数、收缩末期最大血流速度/舒张末期最大血流速度水平降低,观察组低于对照组,差异有统计学意义(P<0.05)。观察组孕妇妊娠不良结局、不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:低分子肝素治疗胎儿生长受限效果较好,可改善胎儿生长发育指标及血流动力学指标,降低妊娠不良结局发生率,改善母体凝血功能。
Objective:To analyze the effect of low molecular weight heparin in the treatment of fetal growth restriction.Methods:Seventy pregnant women with fetal growth restriction admitted to Huizhou Central People's Hospital and Huizhou First People's Hospital from April 2021 to April 2022 were selected as the study objects.According to random number table method,the patients were divided into control group and observation group,with thirty-five cases each.The control group received traditional treat⁃ment,and the observation group received low molecular weight heparin treatment on the basis of control group.Fetal growth and development indexes,maternal uterine height,blood coagulation function,placental hemodynamic indexes,adverse pregnancy out⁃comes and adverse reactions were compared between the two groups.Results:After 2 weeks of treatment,fetal abdominal circum⁃ference,head circumference,femur length and maternal uterine height increased in both groups,and the observation group was larger than the control group,the difference was statistically significant(P<0.05).After 2 weeks of treatment,the levels of acti⁃vated partial thromboplastin time(APTT)and prothrombin time(PT)were increased,and the levels of D-dimer(D-D)and fibrino⁃gen(FIB)were decreased in both groups,APTT and PT levels in observation group were higher than those in control group,and D-D and FIB levels were lower than those in control group,the difference was statistically significant(P<0.05).After 2 weeks of treat⁃ment,the levels of fetal umbilical artery pulsation index,resistance index,end-systolic maximum blood flow velocity/end diastolic maximum blood flow velocity decreased in both groups,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes and adverse reactions in the obser⁃vation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Low mo⁃lecular weight heparin is effective in the treatment of fetal growth restriction,which can improve fetal growth and development in⁃dexes and hemodynamic indexes,reduce the incidence of adverse pregnancy outcomes,and improve maternal coagulation function.
