HDL内miR-223、miR-33a/b表达变化与冠状动脉狭窄程度的相关性研究

摘要 目的:探讨从冠状动脉粥样硬化性心脏病(CAD)患者血浆中分离提取的高密度脂蛋白(HDL)内miR-223、miR-33a、miR-33b表达水平变化与冠状动脉狭窄程度的相关性。方法:选择因胸闷、胸痛、心悸不适就诊于本院心内科并行冠状动脉造影检查(CAG)的86例患者,依据CAG结果分为对照组、冠心病(CAD)组,CAD组采用Gensini积分分为轻中度狭窄组、重度狭窄组。通过超速离心分离法提取血浆中HDL,应用实时荧光定量PCR检测各组HDL内miR-223、miR-33a、miR-33b的表达含量变化。采用多因素Logistic回归分析CAD发生的影响因素,应用Spearman相关分析法分析CAD患者HDL中miRNA表达与Gensini积分的相关性;并绘制ROC评价HDL内miRNA表达水平对CAD诊断价值。结果:CAD组HDL-miR-223表达量明显高于对照组,HDL-miR-33a表达量低于对照组,且CAD重度狭窄组HDL-miR-223相对表达量高于轻中度狭窄组。CAD患者HDL-miR-223相对表达量与Gensini积分呈正相关;ROC分析显示HDL-miR-223、HDL-miR-33a均对CAD的诊断具有一定预测价值,且二者联合对诊断CAD预测价值更高。结论:HDL内miR-223、miR-33a与CAD发生发展密切相关,随着冠脉狭窄程度进展,HDL内miR-223表达量升高。 Objective To investigate the correlation between the expression levels of miR-223,miR-33a and miR-33b in high density lipoprotein(HDL)isolated from plasma and the degree of coronary artery stenosis.Methods A total of 86 patients who were admitted to the cardiology department of our hospital for chest tightness,chest pain and palpitation were selected to undergo coronary angiography(CAG)and divided into control group and coronary heart disease(CAD)group according to CAG results.The CAD group CAD group further categorized into mild-to-moderate stenosis group and severe stenosis group based on Gensini scores.HDL was extracted from plasma by ultracentrifugation,and the expression levels of miR-223,miR-33a and miR-33b in HDL of each group were detected by real-time fluorescent quantitative PCR.Multivariate Logistic regression was used to analyze the influencing factors of CAD,and Spearman correlation analysis was used to analyze the correlation between miRNA expression in HDL and Gensini score in CAD patients.ROC was also plotted to evaluate the value of HDL miRNA expression in CAD diagnosis.Results The expression level of HDL-miR-223 in the CAD group was significantly higher than that in the control group,while the expression level of HDL-miR-33a was lower than that in the control group.Additionally,in the CAD severe stenosis group,the relative expression level of HDL-miR-223 was higher than that in the mild-to-moderate stenosis group.The relative expression level of HDL-miR-223 in CAD patients was positively correlated with Gensini scores.ROC analysis demonstrated that both HDL-miR-223 and HDL-miR-33a had certain predictive value for CAD diagnosis,and their combination had a higher predictive value for diagnosing CAD.Conclusion miR-223 and miR-33a in HDL are closely related to the occurrence and development of CAD,and the expression of miR-223 in HDL increases with the progression of coronary artery stenosis.
