摘要
目的探讨可溶性基质裂解素2(soluble suppression of tumorigenicity 2,sST2)、半乳糖凝集素3(galectin-3,Gal-3)、氨基酸B型利钠肽前体(NT-proBNP)联合检测对心力衰竭(heart failure,HF)患者近期预后的预测价值。方法选取2021年2月至2021年10月就诊于锦州医科大学附属第一医院的HF患者144例,其中男性84例,女性60例。收集患者资料。采用酶联免疫吸附法测定血清sST2、Gal-3水平,采用发光法测定NT-proBNP水平。患者出院后随访半年,根据是否发生不良心血管事件(major adverse cardiovascular events,MACE)分为MACE组40例、非MACE组104例。应用Logistic多因素分析HF患者发生MACE的独立危险因素。应用ROC曲线评估sST2、Gal-3、NT-proBNP联合检测对心衰患者近期预后的预测价值。结果HF患者发生MACE的独立危险因素包括LVEF(P=0.037,OR=0.901,95%CI=0.818~0.994)、NT-proBNP(P=0.032,OR=2.087,95%CI=1.067~4.081)、sST2(P=0.030,OR=1.046,95%CI=1.004~1.089)、Gal-3(P=0.038,OR=1.062,95%CI=1.003~1.124)。相关性分析显示,sST2、Gal-3与LVEF成负相关(r_(s)=-0.660,P<0.05;r s=-0.723,P<0.05),与NT-proBNP成正相关(r s=0.586,P<0.05;r_(s)=0.539,P<0.05);进一步分析显示,sST2与Gal-3成正相关(r s=0.668,P<0.05),差异具有统计学意义。sST2、Gal-3对心衰患者预后均有一定的预测价值,sST2、Gal-3、NT-proBNP三者联合检测可提高预测价值。结论sST2、Gal-3是HF患者发生MACE的独立危险因素,对预测HF近期预后具有一定价值,sST2、Gal-3、NT-proBNP三者联合检测可以提高预测价值。
Objective To investigate the predictive value of combined detection of soluble Suppression of Tumorigenicity 2(sST2),Galectin-3(Gal-3)and NT-proBNP in patients with heart failure(HF).Methods A total of 144 patients with HF,including 84 males and 60 females,were selected from the First Affiliated Hospital of Jinzhou Medical University between February 2021 and October 2021.Clinical data were collected.Serum levels of sST2 and Gal-3 were measured by Elisa and NT-proBNP was measured by luminescence.The patients were followed up for 6 months after discharge,and the patients were divided into MACE group(n=40)and non-MACE group(n=104)according to the occurrence of Major Adverse Cardiovascular Events(MACE).The independent risk factors of MACE in patients with HF were analyzed by Logistic multivariate analysis.ROC curve was used to evaluate the predictive value of combined detection of sST2,Gal-3 and NT-proBNP for short-term prognosis in patients with heart failure.Results The levels of NT-proBNP,sST2 and Gal-3 increased with the decrease of LVEF(P<0.05).Independent risk factors for MACE in patients with HF included LVEF(P=0.037,OR=0.901,95%CI=0.818~0.994)、NT-proBNP(P=0.032,OR=2.087,95%CI=1.067~4.081)、sST2(P=0.030,OR=1.046,95%CI=1.004~1.089)、Gal-3(P=0.038,OR=1.062,95%CI=1.003~1.124).Correlation analysis showed that sST2 and Gal-3 were negatively correlated with LVEF(r_(s)=-0.660,P<0.05;r_(s)=-0.723,P<0.05)and positively correlated with NT-proBNP r_(s)=0.586,P<0.05;r_(s)=0.539,P<0.05,there was a positive correlation between sST2 and Gal-3(r_(s)=0.668,P<0.05).sST2,Gal-3 have some predictive value for the prognosis of patients with HF.Combined detection of sST2,Gal-3 and NT-proBNP can improve the predictive value.Conclusion sST2 and Gal-3 are independent risk factors of MACE in patients with HF,and they are valuable in predicting the short-term prognosis of HF.Combined detection of sST2,Gal-3 and NT-proBNP can improve the predictive value.
