摘要
目的:探讨显微镜下一次性根管治疗对牙隐裂导致的牙髓炎患者的临床疗效及对血清炎性因子的影响。方法:将2021年11月~2022年10月收治的150例牙隐裂所致的牙髓炎患者按就诊顺序进行分组,奇数序号的75例为对照组,偶数序号的75例为观察组。对照组行常规一次性根管治疗,观察组在对照组的基础上加用显微镜进行辅助。对比两组治疗效果、疼痛情况、并发症发生率以及治疗前后炎性因子IL-4、IL-6、TNF-α水平。结果:观察组治疗总有效率98.67%,与对照组的89.33%比较,差异具有统计学意义(P<0.05);观察组Mohd Sulong疼痛分级分布情况优于对照组,差异具有统计学意义(P<0.05);观察组并发症发生率2.67%,与对照组的12.00%比较,差异具有统计学意义(P<0.05);两组治疗前IL-4、IL-6及TNF-α水平比较差异无统计学意义(P>0.05),经治疗后各指标均有不同程度的降低,且两组之间比较差异具有统计学意义(P<0.05)。结论:显微镜下一次性根管治疗能有效提升牙隐裂导致的牙髓炎患者的临床疗效,并减轻疼痛和降低并发症发生率,改善炎性因子水平。
Objective:To investigate the clinical effect of one-time root canal treatment under microscope on pulpitis caused by dental cleft and its effect on serum inflammatory factors.Methods:150 patients with pulpitis caused by dental cryptofissure admitted from November 2021 to October 2022 were divided into groups according to the order of treatment.75 patients with odd number were the control group and 75 patients with even number were the observation group.The control group received routine one-time root canal therapy,and the observation group was assisted by microscope on the basis of control group.The therapeutic effect,pain,incidence of complications,and levels of inflammatory cytokines IL-4,IL-6 and TNF-αbefore and after treatment were compared between the two groups.Results:The total effective rate of the observation group was 98.67%,compared with 89.33%of the control group,the difference was statistically significant(P<0.05).The pain grading distribution of Mohd Sulong in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 2.67%,compared with 12.00%in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the levels of IL-4,IL-6 and TNF-αbetween the two groups before treatment(P>0.05),but after treatment,all indexes were reduced to different degrees,and the difference between the two groups was statistically significant(P<0.05).Conclusion:One-time root canal treatment under microscope can effectively improve the clinical effect of pulpitis caused by dental cleft,reduce pain and complications,and improve the level of inflammatory factors.
