摘要
目的:分析泌尿生殖道感染女性无乳链球菌检出情况及耐药性,指导临床合理使用抗生素。方法:选取2020年6月—2023年6月威海市立医院收治的泌尿生殖道感染女性1660例作为研究对象,采集标本行细菌培养,确认无乳链球菌后进行药敏试验。结果:1660例泌尿生殖道感染女性检出无乳链球菌104例(6.27%),其中产科、妇科、泌尿外科检出率较高。104例乳链球菌标本来源包括阴道分泌物、宫颈分泌物、中段尿,其中阴道分泌物检出率最高。无乳链球菌对青霉素、氨苄西林、万古霉素、利奈唑胺敏感,未出现耐药;对克林霉素、四环素、莫西沙星、左氧氟沙星、环丙沙星的耐药率均>34%,其中对克林霉素、四环素的耐药率较高。结论:泌尿生殖道感染女性无乳链球菌检出率较高,常见于产科、妇科、泌尿外科,阴道分泌物采集可以作为标本采集的主要方式,青霉素、氨苄西林、万古霉素、利奈唑胺可作为治疗无乳链球菌感染的首选药物。
Objective:To analyze the detection and drug resistance of Streptococcus agalactiae in women with urogenital tract infection and guide the rational use of antibiotics in clinical settings.Methods:A total of 1660 women with urogenital tract infection admitted to Weihai Municipal Hospital from June 2020 to June 2023 were selected as the study subjects.Specimens were collected for bacterial culture.After the Streptococcus agalactiae was confirmed,drug susceptible test was performed.Results:Among 1660 women with urogenital tract infection,Streptococcus agalactiae was detected in 104 cases(6.27%).The detection rate was higher in obstetrics department,gynecology department,and urology surgery department.The sources of Streptococcus agalactiae specimens in 104 cases included vaginal discharge,cervical discharge,and midstream urine,among which the detection rate of vaginal discharge was the highest.Streptococcus agalactiae was sensitive to penicillin,ampicillin,vancomycin and linezolid,and had no drug resistance;the drug resistance rates to clindamycin,tetracycline,moxifloxacin,levofloxacin and ciprofloxacin were all>34%,among which the drug resistance rates to clindamycin and tetracycline were higher.Conclusion:The detection rate of Streptococcus agalactiae in women with urogenital tract infection is high,which is common in obstetrics department,gynecology department,and urology surgery department.The vaginal discharge collection can be used as the main method of specimen collection.Penicillin,ampicillin,vancomycin and linezolid can be used as the preferred drugs for the treatment of Streptococcus agalactiae infection.
