不同高度种植钉辅助隐形矫治器内收前牙的三维有限元分析

摘要 目的研究无托槽隐形矫治器用于拔牙后,单纯内收与辅助不同高度微种植钉到尖牙颈部牵引钩200 g牵引力内收时的前牙生物力学变化。方法选取1名牙列完整、身体健康的年轻志愿者,采用锥形束CT建立上颌牙列-无托槽隐形矫治系统三维有限元模型,根据植入微种植钉高度不同分为空白对照组(仅矫治器加力)、3 mm、4 mm、5 mm、6 mm、7 mm、8 mm及9 mm组,均通过微种植钉与尖牙颈部牵引钩200 g牵引力联合器加力;分析各组的前牙位移及牙周膜应力分布情况。结果微种植钉高度≤4 mm时,前牙最大牙周膜等效应力、矢状向倾斜位移随高度增加而增大,切牙的合向位移随高度增加而增加,尖牙的合向位移随高度增加而减小;微种植钉高度≥5 mm时,以上指标均随高度增加而减小。结论无托槽隐形矫治器整体内收前牙时,主张增加种植钉支抗高度≥5 mm,有利于前牙整体位移及压低。 Objective To study the biomechanical changes of the anterior teeth when the invisible appliance without brackets was used for tooth extraction,and when the micro-implant nail with different heights was used for adduction with 200 g traction force between simple adduction and assisted micro-implantation nails with different heights.Methods One young volunteer with complete teeth and healthy condition was selected and their CBCT data was obtained to establish a three-dimensional finite element model of the maxillary dentition-bracketless invisible orthodontic system.The experiment was divided into a blank control group with only aligner force and groups of 3,4,5,6,7,8,and 9 mm depending on the height of the miniscrew implants.All the above groups,except for the height difference,were powered by 200 g of traction and orthodontic appliance force by miniscrew implants with a cervical power arm on the cusp.The trend of displacement of anterior teeth and the change of stress distribution of the periodontal membrane in each experimental group were then analyzed.Results The addition of miniscrew implants could help to counteract the loss of implant anchorage in the overall inward retraction of the anterior teeth in bracketless aligners.When the height of miniscrew implants was≤4 mm,the maximum periodontal equivalent force and sagittally inclined displacement of anterior teeth increased with height.The sympathetic displacement of incisors increased with height,and the sympathetic displacement of cuspids decreased with height;all indicators above≥5 mm decreased with height.Conclusion The height of the implant anchorage is recommended to be increased by≥5 mm when the anterior tooth is fully adduced by the invisible appliance without brackets.Moreover,the increase in height facilitates the overall displacement and depression of the anterior teeth.
