颈动脉CT血管成像和磁共振血管成像在头颈部动脉狭窄的诊断价值分析

Diagnostic Value of Carotid Artery Computed Tomography Angiography (CTA) and MR Angiography (MRA) in Head and Neck Artery Stenosis

摘要 目的对比颈动脉CT血管成像(Computed Tomography Angiography,CTA)和磁共振血管成像(Magnetic Resonance Angiography,MRA)在头颈部疾病中的诊断价值。方法随机选取2022年10月—2023年4月江阴南闸医院收治的60例疑似头颈部动脉狭窄患者为研究对象,所有患者均进行CTA和MRA检查,数字减影血管造影(Digital Subtraction Angiography,DSA)结果作为金标准,共检出中度狭窄及以下患者41例,中度狭窄以上患者19例,对比两种方法的诊断符合率与诊断价值。结果MRA与CTA的准确度分别为95.00%、88.33%,灵敏度分别为95.12%、90.24%,特异度分别为94.74%、84.21%,差异无统计学意义(P均>0.05)。两种诊断方法对动脉狭窄、动脉瘤、脑动静脉畸形(Arteriovenous Malformations,AVM)的疾病诊断符合率比较,差异无统计学意义(P均>0.05)。结论CTA与MRA对头颈部疾病的诊断效果均较为显著,且两种检查方法的诊断价值较高,但MRA略高于CTA。 Objective To compare the diagnostic value of carotid artery computed tomography angiography(CTA) and Magnetic Resonance angiography (MRA) in head and neck diseases. Methods A total of sixty pa⁃tients with suspected head and neck artery stenosis admitted to Jiangyin Nanzha from October 2022 to April2023 were randomly selected as the study objects. CTA and MRA examinations were performed on all patients.Digital subtraction angiography (DSA) was the gold standard. A total of forty-one patients with moderate steno⁃sis or below and nineteen patients with moderate stenosis or above were detected. The diagnostic coincidencerate and diagnostic value of the two methods were compared. Results The accuracy of MRA and CTA were95.00% and 88.33%, the sensitivity was 95.12% and 90.24%, and the specificity was 94.74% and 84.21%, re⁃spectively, and the difference was not statistically significant(all P>0.05). There was no significant difference inthe diagnostic conformity rate of artery stenosis, aneurysm and arteriovenous malformations (AVM) between theto diagnotic methods (all P>0.05). Conclusion Both CTA and MRA have significant diagnostic effects on headand neck diseases, and the diagnostic value of the two methods is higher, but MRA is slightly higher than CTA.
