摘要
目的:探索血清可溶性致癌抑制因子2(sST2)、趋化因子配体20(CCL20)水平对脊柱骨折合并脊髓损伤患者预后的预测价值。方法:选取2020年1月—2022年5月在本院采用椎管减压手术治疗的117例脊柱骨折合并脊髓损伤患者作为疾病组,同期选择采用椎管减压手术治疗的112例单纯脊柱骨折患者作为对照组。检测两组患者血清sST2、CCL20的表达水平,对疾病组患者进行1年的随访,根据预后情况将疾病组患者分为预后良好组(69例)及预后不良组(48例),收集患者的临床资料进行分析;采用多因素Logistic回归分析脊柱骨折合并脊髓损伤患者预后不良的影响因素;受试者工作特征(ROC)曲线分析血清sST2、CCL20检测对脊柱骨折合并脊髓损伤患者预后不良的预测价值。结果:疾病组患者血清sST2、CCL20的表达水平高于对照组患者(P<0.05);预后不良患者血清sST2、CCL20表达水平高于预后良好患者(P<0.05);血清sST2、CCL20是脊柱骨折合并脊髓损伤患者预后不良的影响因素(P<0.05);sST2、CCL20联合预测脊柱骨折合并脊髓损伤患者预后不良的曲线下面积(AUC)显著大于sST2、CCL20单独诊断的AUC(P<0.05),其敏感度为95.83%,特异度为86.96%。结论:脊柱骨折合并脊髓损伤患者血清sST2、CCL20表达水平升高,是患者预后不良的影响因素,sST2、CCL20联合检测能够预测脊柱骨折合并脊髓损伤预后不良的风险。
Objective:To explore the predictive value of serum soluble suppression of tumorigenicity 2(sST2)and chemokine C-C motif ligand 20(CCL20)levels on the prognosis of patients with spinal fracture and spinal cord injury.Methods:A total of 117 patients with spinal fractures and spinal cord injuries who underwent spinal canal decompression surgery in our hospital from January 2020 to May 2022 were regarded as the disease group,112 patients with simple spinal fractures who underwent spinal canal decompression surgery were collected as the control group.The expression levels of sST2 and CCL20 in the serum of patients in two groups was tested,and the disease group patients was followed up for one year,according to the prognosis,patients in the disease group was divided into a good prognosis group(69 cases)and a poor prognosis group(48 cases),and collect clinical data for analysis;multivariate Logistic regression was applied to analyze the influencing factors of poor prognosis in patients with spinal fractures and spinal cord injuries;receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum sST2 and CCL20 detection for poor prognosis in patients with spinal fracture and spinal cord injury.Results:The expression levels of serum sST2 and CCL20 in the disease group were higher than those in the control group(P<0.05);the expression levels of serum sST2 and CCL20 in patients with poor prognosis were higher than those in patients with good prognosis(P<0.05);serum sST2 and CCL20 were the influencing factors for poor prognosis in patients with spinal fracture and spinal cord injury(P<0.05);the area under the curve(AUC)of combined prediction of sST2 and CCL20 for poor prognosis in patients with spinal fracture and spinal cord injury was obviously greater than that of sST2 and CCL20 alone(P<0.05),its sensitivity is 95.83%,and its specificity is 86.96%.Conclusion:The expression levels of serum sST2 and CCL20 in patients with spinal fracture and spinal cord injury are elevated,they are influencing factors for poor prognosis.The combined detection of sST2 and CCL20 can predict the risk of poor prognosis in spinal fracture and spinal cord injury.
