摘要
目的探讨低剂量CT灌注成像参数及血清微小RNA-144-3p(miR-144-3p)、微小RNA-378(miR-378)水平联合检测对早期肺癌的诊断价值。方法回顾性选取2021年8月至2023年8月河南大学第一附属医院收治的早期肺癌患者80例,纳入观察组,另选取同期良性肺病患者80例,纳入对照组。比较两组低剂量CT灌注成像参数[血容积(BV)、血流量(BF)、平均通过时间(MTT)、通透性(PMB)]及血清miR-144-3p、miR-378水平,分析其联合诊断价值及不同水平发生早期肺癌的危险度。结果观察组BV、BF、MTT、PMB值及血清miR-378水平高于对照组,血清miR-144-3p水平低于对照组(P<0.05);BV、BF、MTT、PMB、miR-144-3p、miR-378联合诊断早期肺癌的AUC为0.896,最佳诊断敏感度为90.00%,最佳特异度为87.50%,约登指数为0.775,且高水平患者发生早期肺癌的危险度是低水平的2.333倍、2.810倍、2.478倍、1.857倍、0.509倍、1.759倍(P<0.05)。结论低剂量CT灌注成像参数及血清miR-144-3p、miR-378与早期肺癌发生密切相关,联合检测具有一定诊断效能,且不同水平发生早期肺癌的危险度存在差异,可作为临床诊断早期肺癌的有效方式。
Objective To explore the diagnostic value of low-dose CT perfusion imaging parameters and combined detection of serum microRNA-144-3p(miR-144-3p)and microRNA-378(miR-378)levels for early lung cancer.Methods A retrospective selection was conducted on 80 patients with early lung cancer admitted to the First Affiliated Hospital of Henan University from August 2021 to August 2023,who were included in the observation group.In addition,80 patients with benign lung disease during the same period were selected and included in the control group.The parameters of low-dose CT perfusion imaging[blood volume(BV),blood flow(BF),mean transit time(MTT),permeability(PMB)]and serum levels of miR-144-3p and miR-378 between the two groups were compared,and their combined diagnostic value and the risk of early lung cancer at different levels were analyzed.Results The BV,BF,MTT,PMB values and serum miR-378 levels in the observation group were higher than those in the control group,while serum miR-144-3p levels were lower than those in the control group(P<0.05);The AUC for the combined diagnosis of early lung cancer with BV,BF,MTT,PMB,miR-144-3p,and miR-378 was 0.896,with the optimal diagnostic sensitivity of 90.00%,the optimal specificity of 87.50%,and the Youden index of 0.775.Moreover,the risk of early lung cancer in high-level patients was 2.333 times,2.810 times,2.478 times,1.857 times,0.509 times,and 1.759 times as much as that in low-level patients(P<0.05).Conclusion Low-dose CT perfusion imaging parameters and serum miR-144-3p and miR-378 are closely related to the occurrence of early lung cancer.Combined detection has certain diagnostic efficacy,and the risk of early lung cancer varies at different levels.It can be an effective method for clinical diagnosis of early lung cancer.
