双低剂量冠状动脉CTA应用价值研究

摘要 目的:探讨使用低辐射剂量、低对比剂剂量的双低技术方案在第三代双源CT冠状动脉成像中对图像质量及辐射剂量的影响。方法:收集扬州大学附属医院2023年7月—2023年12月行冠状动脉计算机体层血管成像(CTA)检查的70例患者,随机分为低管电压70 kVp图像(A组)与120 kVp正常管电压(B组)两组进行比较。结果:A组计量长度乘积(DLP)、有效剂量(ED)、容积CT剂量指数(CTDI vol)低于B组,差异均具有统计学意义(P<0.05),A组平均碘摄入量低于B组,差异具有统计学意义(P<0.05)。两组图像质量主观评分差异无统计学意义(P>0.05)。两组患者血管噪声、信噪比(SNR)及对比度噪声比(CNR)在冠状动脉左主干差异均有统计学意义(P<0.05);两组患者血管CT值、噪声、SNR及CNR在右冠状动脉、左前降支及左回旋支差异均无统计学意义(P>0.05)。结论:双低冠状动脉CTA技术可在保证影像质量的前提下大幅度减少病人的辐射剂量及对比剂用量。 Objective To investigate the effect of low-dose and low-contrast technique on image quality and radiation dose in the third generation dual-source CT coronary imaging.Methods 70 patients with coronary artery computed tomography angiography(CTA)from the Affiliated Hospital of Yangzhou University from July 2023 to December 2023 were collected as research objects and randomly divided into two groups with low tube voltage of 70 kVp images(group A)and 120 kVp normal tube voltage(group B)for comparison.Results The values of dose length product(DLP),effective dose(ED)and volume CT dose index(CTDI vol)in group A were significantly lower than those in group B,and the differences were statistically significant(P<0.05),and the average iodine intake of group A was lower than that of group B,with statistical significance(P<0.05).There was no significant difference in the subjective score of image quality between the two groups(P>0.05).There were statistically significant differences in vascular noise,SNR and CNR in the left coronary trunk between the two groups(P<0.05),while there were no significant differences in vascular CT value,noise,SNR and CNR in right coronary artery,left anterior descending artery and left circumflex(P>0.05).Conclusion Double low-dose coronary artery CTA technique can greatly reduce the radiation dose and contrast dose of patients while ensuring the image quality.
