摘要
目的分析中青年宫颈癌患者预后的影响因素。方法选取南阳市中心医院2017年1月至2018年4月就诊的126例中青年宫颈癌患者研究,对其进行为期5年的随访,根据预后将其分为生存组(105例)、死亡组(21例),单因素、多因素logistic回归分析中青年宫颈癌患者预后的影响因素。结果生存组年龄、居住地、婚姻状况、文化程度、病理类型、治疗方案、肿瘤直径、子宫内膜受累与死亡组比较无统计学意义(P>0.05);生存组国际妇产科联盟(FIGO)分期、分化程度、组织学分级、淋巴结转移数目、切缘阳性、脉管癌栓与死亡组比较差异显著(P<0.05)。logistic回归分析,FIGO分期、分化程度、组织学分级、淋巴结转移数目、切缘阳性、脉管癌栓是中青年宫颈癌患者死亡的危险因素(P<0.05)。结论中青年宫颈癌患者死亡与FIGO分期、分化程度、组织学分级、淋巴结转移数目、切缘阳性、脉管癌栓有关,应当引起临床重视。
Objective To analyze the prognosis factors of young and middle-aged patients with cervical cancer.Methods A study of 126 young and middle-aged cervical cancer patients from January 2017 to April 2018 was selected and followed up for 5 years.According to the prognosis,they were divided into survival group(105 cases),death group(21 cases),and the prognosis of univariate and multivariate logistic regression analysis.Results There was no significant difference in age,place of residence,marital status,education level,pathological type,treatment regimen,tumor diameter and endometrial involvement between survival group and death group(P>0.05).There were significant differences between survival group and death group in staging,differentiation degree,histological grade,number of lymph node metastasis,positive incisal margin and vascular cancer thrombus(P<0.05).Logistic regression analysis show that FIGO stage,degree of differentiation,histological grade,number of lymph node metastases,positive resection margin,and vascular cancer thrombus were the risk factors for death in young and middle-aged cervical cancer patients,P<0.05.Conclusion The death of young and middle-aged cervical cancer patients is related to FIGO stage,differentiation degree,histological grade,number of lymph node metastases,positive resection margin,and vascular cancer thrombus,which should be paid clinical attention.
