摘要
目的:利用CT血管成像(CTA)评估股浅动脉(SFA)狭窄程度和侧支循环形成情况,及当SFA显著性狭窄后从股深动脉(DFA)和旋股外侧动脉降支(DBLCFA)发展而来的侧支系统。方法:选择2019年1月—2023年6月本院接受下肢动脉CTA检查且双侧SFA显著性狭窄的患者。使用已建立的评分系统测量SFA狭窄程度、侧支血管数量及大小,再比较从DFA和DBLCFA发展而来的侧支数量。结果:随着SFA狭窄程度的增加,在CTA上更有可能看到更大、更多的侧支血管。DFA发展的侧支血管多于DBLCFA的侧支形成。当股浅动脉显著性狭窄后,有8.1%的患者的侧支血管仅来自DBLCFA。结论:当计划制取股前外侧皮瓣时,最好对患者进行术前CTA评估侧支数量及大小,了解这些对临床皮瓣的选择具有重要意义。
Objective:To assess the degree of stenosis and collateral circulation in the superficial femoral artery(SFA)using CTA,and the collateral systems developed from deep femoral artery(DFA)and descending branch of lateral circumflex femoral artery(DBLCFA)after significant stenosis of SFA.Methods:Patients who underwent CTA examination of lower extremity arteries in our hospital from January 2019 to June 2023 and had significant bilateral SFA stenosis were selected.The degree of SFA stenosis and the number and size of collateral vessels were measured using the established scoring systems.Then the number of collateral vessels developed from DFA and DBLCFA was compared.Results:As the severity of SFA stenosis increased,larger and more collateral vessels were more likely to be seen on CTA.The collateral vessels developed by DFA were more than those formed by DBLCFA.After significant SFA stenosis,only 8.1%of patients had collateral vessels from DBLCFA alone.Conclusion:When planning to produce an anterolateral thigh flap,it is advisable to perform preoperative CTA in the patient to assess the number and size of collateral vessels,which are important for clinical flap selection.
