摘要
目的探讨小柴胡汤加减结合左西替利嗪治疗急性荨麻疹的效果及对炎症因子、免疫因子及变态反应的影响。方法选择2019年7月至2022年7月收治的120例急性荨麻疹患者作为研究对象,采用随机数字表法将其分为对照组和观察组,各60例。对照组给予左西替利嗪治疗,观察组给予小柴胡汤加减结合左西替利嗪治疗。比较两组的症状改善情况、炎症因子、免疫因子水平及变态反应指标。结果治疗前,两组的瘙痒程度、风团数量、风团发作频率及风团持续时间评分比较,差异无统计学意义(P>0.05);治疗后,两组的瘙痒程度、风团数量、风团发作频率及风团持续时间评分均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05);治疗后,两组的IL-6、TNF-α水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的总免疫球蛋白E(tIgE)、嗜酸性粒细胞阳离子蛋白(ECP)及分泌型免疫球蛋白E(sIgE)水平比较,差异无统计学意义(P>0.05);治疗后,两组的tIgE、ECP及sIgE水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的活化T细胞趋化因子(RANTES)、单核细胞趋化蛋白-1(MCP-1)水平比较,差异无统计学意义(P>0.05);治疗后,两组的RANTES、MCP-1水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。结论小柴胡汤加减结合左西替利嗪治疗急性荨麻疹患者可有效缓解症状,改善炎症因子、免疫因子及变态反应指标水平。
Objective To investigate the effect of modified Xiaochaihu decoction combined with levocetirizine in the treatment of acute urticaria and its influences on inflammatory factors,immune factors and allergic reactions.Methods A total of 120 patients with acute urticaria who were treated from July 2019 to July 2022 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 60 cases in each group.The control group was treated with levocetirizine,and the observation group was treated with modified Xiaochaihu decoction combined with levocetirizine.The improvement of symptoms,levels of inflammatory factors,immune factors and allergic reaction indexes were compared between the two groups.Results Before treatment,there were no significant differences in the scores of pruritus degree,number of wheals,frequency of wheals and duration of wheals between the two groups(P>0.05);after treatment,the scores of pruritus degree,number of wheals,frequency of wheals and duration of wheals in the two groups decreased,and those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)between the two groups(P>0.05);after treatment,the levels of IL-6 and TNF-α in the two groups decreased,and those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of total immunoglobulin E(tIgE),eosinophil cationic protein(ECP)and secretory immunoglobulin E(sIgE)between the two groups(P>0.05);after treatment,the levels of tIgE,ECP and sIgE in the two groups decreased,and those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of regulated upon activation normal T cell expressed and secreted(RANTES)and monocyte chemoattractant protein-1(MCP-1)between the two groups(P>0.05);after treatment,the levels of RANTES and MCP-1 in the two groups decreased,and those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).Conclusion Modified Xiaochaihu decoction combined with levocetirizine in the treatment of patients with acute urticaria can effectively relieve symptoms,improve the levels of inflammatory factors,immune factors and allergic reaction indexes.
