摘要
目的:分析孕早期孕妇采用胎盘生长因子(PIGF)联合平均动脉压(MAP)、子宫动脉搏动指数(UtPI)预测子痫前期(PE)的临床价值.方法:选取2022年11月—2023年8月于河西学院附属张掖人民医院妇产科行产前检查的孕早期孕妇500例为研究对象,经临床检查证实PE者117例,将其作为观察组,其余383例健康孕妇作为对照组.测定并比较两组PIGF、MAP、UtPI水平,采用受试者工作曲线(ROC)评价各个指标单一及联合检测对PE的预测效能.结果:观察组PlGF水平低于对照组,MAP、UtPI水平高于对照组,差异有统计学意义(P<0.001).PIGF、MAP、UtPI单项及联合检测预测PE的ROC曲线下面积分别是0.755、0.742、0.756、0.893,且联合检测的敏感度最高.结论:PIGF、MAP、UtPI在预测孕早期孕妇PE方面均具有较高的临床价值,且三项指标联合的预测效能优于各单项指标.
Objective:To analyze the clinical value of predicting preeclampsia(PE)using placental growth factor(PIGF)combined with mean arterial pressure(MAP)and uterine artery pulsatility index(UtPI)in pregnant women in early pregnancy.Methods:Five hundred cases of pregnant women in early pregnancy who underwent prenatal examination in the Department of Obstetrics and Gynecology of Zhangye People's Hospital Affiliated to Hexi College from November 2022 to August 2023 were selected as the study subjects,one hundred and seventeen cases of PE confirmed by clinical examination were taken as the observation group,and the remaining three hundred and eighty-three cases of healthy pregnant women were taken as the control group.The levels of PIGF,MAP and UtPI were measured and compared between the two groups,and the predictive efficacy of single and combined tests for PE was evaluated by using the subject's working curve(ROC).Results:The PlGF level of the observation group was lower than that of the control group,and the levels of MAP and UtPI were higher than that of the control group,with statistically significant differences(P<0.001).The areas under the ROC curves for the prediction of PE by single and combined tests for PIGF,MAP,and UtPI were 0.755,0.742,0.756,and 0.893,respectively,and the sensitivity of combined tests was the highest.Conclusion:PIGF,MAP,and UtPI all have high clinical value in predicting PE in pregnant women in early pregnancy,and the predictive efficacy of the combination of the three indicators is better than that of each single indicator.
