摘要
目的探究经皮雌二醇凝胶联合穴位贴敷对胚胎停育患者人工流产术后子宫内膜的修复效果。方法选取江苏省无锡市妇幼保健院2021年1月至2023年5月收治的82例拟行人工流产手术的胚胎停育患者为研究对象,依据随机数字表法将其分成两组,各41例。对照组给予经皮雌二醇凝胶,观察组给予经皮雌二醇凝胶联合穴位贴敷,连续治疗4周。比较两组治疗前,治疗2、4周后子宫内膜厚度;比较两组治疗前后卵泡刺激素(FSH)、雌二醇(E_(2))、黄体生成素(LH)、人绒毛促性腺激素(HCG)水平;比较两组治疗前,治疗3、5 d时视觉模拟评分法(VAS)评分;比较两组月经恢复情况;记录两组不良反应及术后并发症的发生情况。结果整体分析发现:两组子宫内膜厚度时间、组间、交互作用比较,差异有统计学意义(P<0.05)。进一步两两比较,组内比较:治疗2、4周后,两组子宫内膜厚度高于治疗前(P<0.05);治疗4周后,两组子宫内膜厚度高于治疗2周后(P<0.05)。组间比较:治疗2、4周后,观察组子宫内膜厚度高于对照组(P<0.05)。治疗后,两组FSH、E_(2)、LH水平高于治疗前,HCG水平低于治疗前(P<0.05);观察组FSH、E_(2)、LH水平高于对照组,HCG水平低于对照组(P<0.05)。整体分析发现:两组VAS评分时间、组间、交互作用比较,差异有统计学意义(P<0.05)。进一步两两比较,组内比较:治疗3、5 d时,两组VAS评分低于治疗前(P<0.05);治疗5 d时,两组VAS评分低于治疗3 d时(P<0.05)。组间比较:治疗3、5 d时,观察组VAS评分低于对照组(P<0.05)。观察组月经复常率高于对照组,复潮时间短于对照组,月经量减少率低于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。两组术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论经皮雌二醇凝胶联合穴位贴敷用于胚胎停育患者人工流产术后可有效促进子宫内膜恢复,改善内分泌激素水平,减轻疼痛,且用药较为安全,术后并发症及不良反应较少。
Objective To explore the effect of percutaneous Estradiol Gel combined with acupoint application on endometrium repair after induced abortion in patients with embryo termination.Methods A total of 82 patients with embryo termination who were expected to undergo induced abortion admitted to Wuxi Maternal and Child Health Hospital of Jiangsu Province from January 2021 to May 2023 as the study objects.They were divided into two groups by random number table method,with 41 cases in each groups.The control group was given percutaneous Estradiol Gel,and observation group was given percutaneous Estradiol Gel combined with acupoint application,continuous treatment for four weeks.The endometrial thickness of two groups was compared before treatment and after two and four weeks of treatment;the levels of follicle stimulating hormone(FSH),estradiol(E_(2)),luteinizing hormone(LH),and human chorionic gonadotropin(HCG)before and after treatment were compared between two groups;visual analogue scale(VAS)scores were compared between two groups before treatment and three and five days of treatment;the recovery of menstruation was compared between two groups;the occurrence of adverse reactions and postoperative complications were recorded in two groups.Results Overall analysis showed that there were significant differences in endometrial thickness time,intergroup,and interaction between two groups(P<0.05).Further pairwise comparison,intra-group comparison:after two and four weeks of treatment,endometrial thickness in two groups were higher than those before treatment(P<0.05);after four weeks of treatment,endometrial thickness in both groups was higher than that after two weeks of treatment(P<0.05).Comparison between groups:after two and four weeks of treatment,endometrial thickness in observation group was higher than that in control group(P<0.05).After treatment,the levels of FSH,E_(2),and LH were higher than those before treatment,and the level of HCG was lower than that before treatment(P<0.05);the levels of FSH,E_(2),and LH in observation group were higher than those in control group,while the level of HCG was lower than that in control group(P<0.05).Overall analysis showed that there were significant differences in VAS score time,inter group,and interaction between two groups(P<0.05).Further pairwise comparison,intra-group comparison:at three and five days of treatment,VAS scores in both groups were lower than those before treatment(P<0.05);at five days of treatment,VAS scores in both groups were lower than those at three days of treatment(P<0.05).Comparison between groups:at three and five days of treatment,VAS scores of observation group were lower than those of control group(P<0.05).The recurrence rate of menstruation in observation group was higher than that in control group,the recurrence time was shorter than that in control group,and the reduction rate of menstrual volume was lower than that in control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between two groups(P>0.05).There was no significant difference in the total incidence of postoperative complications between two groups(P>0.05).Conclusion Percutaneous Estradiol Gel combined with acupoint application can effectively promote endometrial recovery,improve endocrine hormone levels,relieve pain,and be used safe with fewer complications and adverse reactions.
