摘要
目的分析糖化血红蛋白和糖化血清蛋白联合检测在早期妊娠期糖尿病中的诊断价值,旨在为早期妊娠期糖尿病寻求有效的检测指标。方法以198例早期妊娠期糖尿病孕妇作为实验组,再选取同期健康孕妇200例作为对照组,两组均进行糖化血红蛋白和糖化血清蛋白检测。对比两组孕妇各项检测指标的阳性率,对比三种检测方法的灵敏度、特异度、准确性。结果实验组孕妇的糖化血红蛋白、糖化血清蛋白以及联合检测的阳性率分别为83.3%、86.9%、98.0%,明显高于对照组的12.0%、8.0%、2.0%,差异均具有统计学意义(P<0.05)。联合检测的灵敏度、特异度以及准确性分别为98.0%、98.0%、98.0%,明显高于糖化血红蛋白检测的83.3%、88.0%、85.7%和糖化血清蛋白检测的86.9%、92.0%、89.4%,差异具有统计学意义(P<0.05)。结论妊娠期糖尿病在早期可通过联合检测糖化血红蛋白和糖化血清蛋白进行评估和诊断,可显著提高诊断率,具有一定的临床价值。
Objective To analyze the analyze the diagnostic value of combined detection of glycosylated hemoglobin and glycosylated serum protein in the diagnosis of early gestational diabetes mellitus,so as to find effective indicators for detection of early gestational diabetes mellitus.Methods 198 cases of early gestational diabetes mellitus were selected as the experimental group,and 200 healthy pregnant women during the same period were selected as the control group.The glycosylated hemoglobin and glycosylated serum protein were detected in both groups.The positive rates of the two groups of pregnant women were compared,and the sensitivity,specificity and accuracy of the three detection methods were compared.Results The positive rates of glycosylated hemoglobin and glycosylated serum protein alone and in combination in the experimental group were 83.3%,86.9%,and 98.0%,which were significantly higher than 12.0%,8.0%,and 2.0%in the control group,and the differences were all statistically significant(P<0.05).The sensitivity,specificity,and accuracy of the combined detection were 98.0%,98.0%,and 98.0%,which were significantly higher than those of the glycosylated hemoglobin(83.3%,88.0%,and 85.7%)and the glycosylated serum protein(86.9%,92.0%,and 89.4%)alone.The differences were all statistically significant(P<0.05).Conclusion Gestational diabetes mellitus can be evaluated and diagnosed by combined detection of glycosylated hemoglobin and glycosylated serum protein at the early stage,which can significantly improve the diagnostic rate and has certain clinical value.
