摘要
目的:针对肠梗阻的影像学检查,对临床常用的两种检查设备(数字X射线摄影和多层螺旋CT)作临床应用比较。方法:以2020年1月~2023年10月遵义医科大学第三附属医院收治的60例疑似肠梗阻患者为研究对象,所有患者均行数字X射线摄影(DR)和多层螺旋CT(MSCT)检查。对检查患者追踪随访证实后,回顾性分析资料,设立两个比较项,即诊断效能比较:灵敏度、特异度、阴性预测值、阳性预测值、Kappa值;诊断符合率比较:梗阻部位及梗阻性质。结果:MSCT诊断的灵敏度(98.18%)、特异度(80.00%)、阴性预测值(80.00%)、阳性预测值(98.18%)、Kappa值(0.589)较DR诊断肠梗阻的灵敏度(88.88%)、特异度(66.66%)、阴性预测值(40.00%)、阳性预测值(96.00%)、Kappa值(0.287)比较效能更优,有统计学差异(P<0.05);MSCT检查梗阻部位及梗阻性质判断符合率显著高于DR(P<0.05)。结论:MSCT对肠梗阻的临床应用效能优于DR,MSCT在肠梗阻部位、性质等方面诊断效果优于DR。临床医生可根据患者的具体情况和临床需求综合考虑。
Objective:To compare the clinical application of two commonly used diagnostic devices(DR and MSCT)for imaging examination of intestinal obstruction.Methods:60 suspected patients with intestinal obstruction admitted to the Third Affiliated Hospital of Zunyi Medical University from January 2020 to October 2023 were selected as the study subjects.All patients underwent digital X-ray imaging(DR)and multi-slice spiral CT(MSCT)examination.After confirming the follow-up of the examined patients,a retrospective analysis of the data was conducted and two comparison items were established:diagnostic efficacy comparison:sensitivity,specificity,negative predictive value,positive predictive value,and Kappa value comparison of diagnostic accuracy:location and nature of obstruction.Results:The sensitivity(98.18%),specificity(80.00%),negative predictive value(80.00%),positive predictive value(98.18%),and Kappa value(0.589)of MSCT diagnosis were more effective than DR diagnosis of intestinal obstruction(88.88%),specificity(66.66%),negative predictive value(40.00%),positive predictive value(96.00%),and Kappa value(0.287),with statistical differences(P<0.05);The accuracy of MSCT in determining the location and nature of obstruction was significantly higher than DR(P<0.05).Conclusion:The clinical efficacy of MSCT in the treatment of intestinal obstruction is superior to DR,and the diagnostic effect of MSCT in the location and nature of intestinal obstruction is superior to DR.Clinical doctors can comprehensively consider the specific situation and clinical needs of patients.
