摘要
目的:探讨负压封闭引流技术(Vacuum sealing drainage,VSD)联合Masquelet技术对老年创伤性粉碎性骨折患者的疗效及对炎性因子与感染控制情况的影响。方法:选择2021年1月~2022年12月本院收治的60例老年创伤性粉碎性骨折患者作为研究对象,随机数字表法将其分为两组,对照组30例行常规治疗,研究组加用VSD联合Masquelet技术,对比两组治疗效果、肿瘤坏死因子α(tumornecrosisfactorα,TNF-α)、C反应蛋白(C-reactive protein,CRP)、白细胞介素6(interleukin 6,IL-6)等炎性因子控制水平、感染控制情况等。结果:研究组治疗效果高于对照组(P<0.05);两组治疗后炎性因子水平均低于治疗前(P<0.05),治疗后研究组TNF-α、CRP及IL-6水平均低于对照组(P<0.05),研究组感染控制率为96.67%(29/30)高于对照组的63.33%(19/30),差异有统计学意义(P<0.05),研究组并发症发生率为3.33%(1/30)低于对照组的36.67%(11/30),差异有统计学意义(P<0.05)。结论:VSD联合Masquelet技术在老年创伤性粉碎性骨折患者中的治疗效果显著,患者经过治疗后炎性因子水平显著降低,患者感染控制情况得到改善,并发率发生率降低,说明联合疗法具有较高临床应用价值。
Objective:To investigate the therapeutic effect of Vacuum sealing drainage(VSD)and sealing drainage(Masquelet)in the treatment of senile patients with traumatic comminuted fractures and the effect of the sealing drainage on inflammatory factors and infection control.Methods:60 elderly patients with traumatic comminuted fractures admitted to our hospital from January 2021 to December 2022 were selected as the study subjects,and they were randomly divided into two groups using a random number table method.The control group received routine treatment for 30 cases,while the study group received VSD combined with Masquelet technique.The treatment effects,tumor necrosis factor alpha(TNF-α),C-reactive protein(CRP),interleukin-6(IL-6)and other inflammatory factors control levels,as well as infection control,were compared between the two groups.Results:The treatment effect of the study group was higher than that of the control group(P<0.05);The levels of inflammatory factors in both groups after treatment were lower than before treatment(P<0.05);After treatment,the average levels of TNF-α,CRP,and IL-6 in the study group were lower than those in the control group(P<0.05);The infection control rate of the study group was 96.67%(29/30),which was higher than that of the control group's 63.33%(19/30),and the difference was statistically significant(P<0.05);The incidence of complications in the study group was 3.33%(1/30),which was lower than that of the control group's 36.67%(11/30),and the difference was statistically significant(P<0.05).Conclusion:VSD combined with Masquelet has significant therapeutic effect in elderly patients with traumatic comminted fracture.The levels of inflammatory factors are significantly reduced after treatment,the infection control of patients is improved,and the incidence of complications is reduced,indicating that combined therapy has high clinical application value.
