摘要
目的:分析老年桥本甲状腺炎(HT)发生肝损伤与患者自身抗体促甲状腺激素受体抗体(TRAb)、甲状腺球蛋白抗体(TGAb)、过氧化物酶抗体(TPOAb)水平之间的关系。方法:选取2019年1月至2023年10月郑州市中医院老年HT患者60例,根据是否发生肝损伤分为损伤组21例、无损伤组39例。单因素分析法比较两组临床资料差异;Logistic回归分析法分析TRAb、TGAb、TPOAb水平与肝损伤相关性。结果:与无损伤组比较,损伤组游离甲状腺素(FT4)、免疫球蛋白A(IgA)、促甲状腺激素(TSH)、TRAb、TGAb、TPOAb水平升高,差异均有统计学意义(P<0.05)。Logistic回归分析显示,FT4、TRAb、TGAb、TPOAb水平升高均是肝损伤的危险因素(P<0.05)。结论:老年HT患者自身抗体TRAb、TGAb、TPOAb水平高是发生肝损伤的危险因素,临床治疗老年HT患者时,注意预防肝损伤。
Objective:The relationship between the liver injury in elderly hashimoto thyroiditis(HT)and the levels of autoantibody thyroid-stimulating hormone receptor antibody(TRAb),thyroglobulin antibody(TGAb),and peroxidase antibody(TPOAb)were analyzed.Methods:A total of 60 elderly patients with HT in Zhengzhou Hospital of Traditional Chinese Medicine from January 2019 to October 2023 were selected and divided into injury group and non-injury group according to whether liver injury occurred.Compare the differences between two groups using single factor analysis method;Logistic regression analysis was used to analyze the correlation between TRAb,TGAb,TPOAb levels and liver injury.Results:Among 60 patients with HT,35.00%were accompanied by liver damage of varying degrees and were set as the injury group,and the remaining 39 cases were set as the non-injury group.Compared with the non-injury group,the levels of free thyroxine(FT4),immunoglobulin A(IgA),thyroid-stimulating hormone(TSH),TRAb,TGAb and TPOAb were significantly increased in the injury group(P<0.05).Logistic regression analysis showed that elevated levels of FT4,TRAb,TGAb,and TPOAb were all risk factors for liver injury(P<0.05).Conclusion:The high levels of autoantibodies TRAb,TGAb,and TPOAb in elderly HT patients are risk factors for liver injury,which can provide a reference for clinical liver protection treatment of the disease.
