摘要
目的 研究北京市通州区巨大儿发生率,分析巨大儿发生的影响因素,为促进妇幼及家庭健康提供依据。方法 利用前瞻性队列(北大-通州出生队列)的产前保健及分娩数据,描述2018-2019年北京市通州区巨大儿的发生情况,采用卡方检验比较不同人群的巨大儿发生率,使用logistic回归模型分析巨大儿发生的影响因素。结果 2018-2019年北京市通州区巨大儿的发生率为8.2%。母亲孕前超重或肥胖、父亲超重或肥胖、有巨大儿分娩史、过期产及男婴的巨大儿发生率较高。logistic回归分析发现,单胎孕妇是否分娩巨大儿的影响因素有新生儿性别、巨大儿分娩史、分娩类型及妇女孕前体型。结论 巨大儿分娩史、新生儿性别、分娩类型及孕妇孕前体型是巨大儿发生的影响因素。应提前预测巨大儿发生风险,加强保健和指导,减少巨大儿发生和巨大儿对母婴的不良影响。
Objective To research the prevalence of macrosomia in Tongzhou District,Beijing,analyze the influencing factors,and provide a basis for promoting maternal,child and family health.Methods Based on the data from Peking University Birth Cohort in Tong-zhou(PKUBC-T),the prevalence of macrosomia in Tongzhou District,Beijing from 2018 to 2019 was described and compared using chi-square test,and the influencing factors were explored using logistic regression model.Results The prevalence of macrosomia in Tongzhou,Beijing from 2018 to 2019 was 8.2%.High prevalence of macrosomia was found among the mothers who are overweight or fat before pregnan-cy,or with delivery history of macrosomia,or post-term birth,or for the fathers who are overweight or fat,or for baby boys.Logistic regres-sion analysis showed that gender of neonates,macrosomia history of mothers,delivery modes,and maternal somatotype before pregnancy were the influencing factors of macrosomia for single births.Conclusion Macrosomia history of mothers,gender of baby,delivery modes,and maternal somatotype before pregnancy are influencing factors for macrosomia.The risk of macrosomia should be predicted ahead,and health care and instruction for women and their families should be reinforced to reduce the prevalence of macrosomia and the harmful effect of macrosomia on mothers and babies.
