摘要
目的 观察妊娠期甲状腺功能减退症(甲减)对妊娠结局影响,评价左甲状腺素钠片治疗的临床价值。方法 选取119例产前检查发现甲减孕妇,按照1∶1比例选取同期正常孕妇119例为正常组;对甲减孕妇给予左甲状腺素钠片替代治疗,观察其治疗效果。结果 119例甲减孕妇中78例孕妇分娩前促甲状腺激素(TSH)<2.5 mIU/L(观察组),41例孕妇TSH>2.5mIU/L(对照组)。三组妊娠及围生儿不良结局比较,差异有统计学意义(P<0.05)。对照组妊娠不良结局发生率48.78%、围生儿不良结局发生率26.83%均分别高于观察组的16.67%、10.26%(P<0.05);正常组与观察组不良结局发生率、围生儿不良结局比较,差异无统计学意义(P>0.05)。结论 妊娠期甲减对母婴结局影响较大,应该早期开展甲状腺功能指标筛查,加大监测力度,尽早及时采取左甲状腺素钠片治疗以降低相应风险。
Objective To observe the effect of hypothyroidism(hypothyroidism) on pregnancy outcome and evaluate the clinical value of levothyroxine sodium tablets.Methods 119 pregnant women were selected as the normal group according to 1:1 ratio.The pregnant women with hypothyroidism were treated with levothyroxine sodium tablets and the therapeutic effect was observed.Results Among 119 pregnant women with hypothyroidism,78 had TSH<2.5 mIU/L before delivery(observation group),and 41 had TSH>2.5 mIU/L(control group).The adverse outcomes of pregnancy and perinatal infants were compared among the three groups,and the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in control group was 48.78% and the incidence of perinatal adverse outcomes in control group was 26.83%,which were higher than those in observation group(16.67% and 10.26%,respectively)(P<0.05).There was no significant difference in the incidence of adverse outcomes and perinatal adverse outcomes between the normal group and the observation group(P>0.05).Conclusion Hypothyroidism during pregnancy has a great impact on maternal and infant outcomes.Screening of thyroid function indicators should be carried out early,monitoring should be intensified,and levothyroxine sodium tablets should be taken as soon as possible to reduce the corresponding risk.
