摘要
目的探讨患者参与式饮食干预(PPDI)在改善胃肠癌术后营养状况中的应用。方法选择2018年10月-2019年9月入住苏州大学附属太仓医院胃肠外科的80例胃肠癌术后患者作为研究对象。采用历史队列研究的方法,将2018年10月-2019年2月的40例作为对照组,将2019年3月-9月的40例作为观察组。对照组予以常规饮食指导和常规随访,观察组在对照组基础上采用PPDI指导患者及家属记录“饮食日记”,与患者共同制订出院后的饮食计划,并细化随访内容和缩短随访间隔,根据随访内容及时调整饮食计划。比较两组患者出院后1周、出院后4周的饮食目标完成情况、人体测量(BMI、握力)、营养指标(血浆蛋白)。结果观察组患者的饮食完成情况、营养指标在出院后4周时与对照组相比表现出明显优势,差异具有统计学意义(P<0.05);两组的人体测量指标差异不明显,差异无统计学意义(P>0.05)。结论PPDI能提高患者的饮食依从性,摄入合适的营养量,改善患者的营养状况。
Objective To investigate the effectiveness of patient participation-based dietary intervention(PPDI)in improving the nutritional status of patients following gastrointestinal cancer surgery.Methods A total of 80 patients with gastrointestinal cancer admitted to the department of Gastrointestinal Surgery at Taicang Affiliated Hospital of Soochow University from October 2018 to September 2019 were included in this study.Using a histori-cal cohort study design,40 patients from October 2018 to February 2019 comprised the control group,while 40 pa-tients from March 2019 to September 2019 formed the observation group.All patients received routine dietary guid-ance and follow-ups,while the observation group additionally received PPDI,involving the collaborative develop-ment of post-discharge dietary plans,refinement of follow-up procedures,and adjustment of dietary plans based on timely feedback.The achievement of dietary goals,anthropometric measurements(BMI,grip strength),and nutri-tional indicators(plasma protein)were compared between the two groups at 1 week and 4 weeks post-discharge.Results At 4 weeks post-discharge,the observation group demonstrated significantly better diet achievement and nutritional indicators compared to the control group,with a significant difference(P<0.05).There was no signifi-cant difference in anthropometric indicators between the two groups(P>0.05).Conclusion Patient participation-based dietary intervention(PPDI)can enhance dietary compliance,ensure appropriate nutrient intake,and improve the nutritional status of patients undergoing gastrointestinal cancer surgery.
