产后盆底肌修复中盆底肌肉锻炼结合生物反馈与电刺激的效果及对ICIQ-SF和FSFI评分的影响

Effect of pelvic floor muscle combined exercise with stimulation electrical and biofeedback in postpartum pelvic floor muscle repair and its influence on ICIQ-SF and FSFI scores

摘要 目的 观察产后盆底肌修复中采用盆底肌肉锻炼结合电刺激与生物反馈的效果及对国际尿失禁问卷调查简表(ICIQ-UI Short Form, ICIQ-SF)和女性性功能指数量表(Female Sexual Function Index, FSFI)评分的影响。方法 选取2022年5月—2023年10月期间至郑州大学第一附属医院产科展开盆底肌修复的120例患者纳入研究,按照不同的干预方式将患者分为研究组和常规组,每组各60例患者。常规组采用盆底肌肉锻炼+生物反馈治疗,研究组则在此基础之上结合电刺激治疗,对比两组患者干预效果。结果 研究组盆底肌力恢复率、子宫脱垂消失率以及尿失禁改善率分别为100.00%、96.67%及93.33%,明显优于常规组(P<0.05);两组患者在治疗之前的最大快肌收缩值、60s和10s慢肌收缩比较无统计学意义(P>0.05);研究组治疗后最大快肌收缩值、60s和10s慢肌收缩指标分别为(38.69±7.79)μV、(26.37±6.64)μV和(32.17±4.28)μV,显著高于常规组(P<0.05);治疗前两组患者ICIQ-SF各项评分比较无统计学意义(P>0.05),研究组治疗后ICIQ-SF尿失禁对生活的影响、漏尿频率、漏尿量及总分均低于常规组(P<0.05);治疗前两组患者FSFI各项评分比较无意义(P>0.05),研究组FSFI性生活的满意度、性高潮、阴道的润滑度、性唤起和性欲评分明显高于常规组,性交疼痛评分低于常规组(P<0.05)。结论 盆底功能障碍采用盆底肌肉锻炼结合生物反馈与电刺激,不仅可以对患者肌纤维进行有效改善,同时还可以促使患者子宫脱垂以及漏尿等不良症状进行缓解,对于性生活质量的提升有显著的效果,值得进行推广。 Objective To observe the effect of pelvic floor muscle exercise combined with electrical stimulation and biofeedback on ICIQ-SF and FSFI scores in postpartum pelvic floor muscle repair.Methods: A total of 120 patients who underwent pelvic floor muscle repair in the Department from February May 2022 to October 2023 were selected to be included in the study. According to different methods intervention, divided the patients into group routine and group study, with 60 patients in each group. The conventional group was treated with pelvic floor muscle exercise and biofeedback therapy, while the study group was treated with electrical stimulation on this basis to compare the intervention effect of the two groups.Results The recovery rate of pelvic floor muscle strength, disappearance rate of uterine prolapse and improvement rate of urinary incontinence in the study group were 100.00%, 96.67% and 93.33%, respectively, which were significantly better than those in the conventional group(P<0.05). There was no statistical significance in the maximum fast muscle contraction and the slow muscle contraction of 60s and 10s between the two groups before treatment(P>0.05). The maximum fast muscle contraction value, slow muscle contraction indexes of 60s and 10s after treatment were(38.69±7.79) μV,(26.37±6.64) μV and(32.17±4.28) μV, respectively, which were significantly higher than those of conventional group(P<0.05).There was no statistical significance in ICIQ-SF scores between the two groups before treatment(P>0.05). The impact of ICIQ-SF incontinence on life, urinary leakage frequency, urinary leakage volume and total score of the study group after treatment were lower than those of the conventional group(P<0.05).Before treatment, the FSFI scores of the two groups were significantly less significant(P>0.05). The scores of FSFI sexual satisfaction, orgasm, vaginal lubrication, sexual arousal and sexual desire in the study group were significantly higher than those in the conventional group, and the score of pain during intercourse was lower than that in the conventional group(P<0.05).Conclusion The use of pelvic floor muscle exercise combined with biofeedback and electrical stimulation for pelvic floor dysfunction can not only effectively improve the muscle fibers of patients, but also alleviate adverse symptoms such as uterine prolapse and urine leakage, which has a significant effect on the improvement of sexual life quality and is worth promoting.
