摘要
目的探讨临床路径教学法联合小型临床评估练习(mini clinical evaluation exercise,Mini-CEX)在神经外科住院医师规范化培训(简称“住培”)教学中的应用效果。方法选取2021年9月—2023年6月哈尔滨医科大学附属第一医院神经外科的32名住培在读研究生作为研究对象,按照随机数字表法将研究生分为常规教学组和临床路径教学组,每组16名。常规教学组采用常规教学法联合Mini-CEX评估法进行教学,临床路径教学组采用常规教学法联合Mini-CEX评估法进行教学。学期末对学员进行理论知识和临床技能考试、Mini-CE评估以及满意度调查,比较2组的教学效果。结果临床路径教学组学员的理论知识考核成绩[(52.74±4.28)分]高于常规教学组[(49.26±3.73)分],差异有统计学意义(P<0.05)。临床路径教学组学员Mini-CEX评估成绩中的沟通技巧[(7.89±0.94)分]、临床判断[(7.16±0.73)分]、组织效能[(8.20±0.75)分]、整体表现[(7.71±0.83)分]评分高于常规教学组[(7.23±0.72)分、(6.54±0.94)分、(7.45±0.93)分、(7.10±0.81)分],差异有统计学意义(P<0.05)。临床路径教学组学员的总满意度为100%,高于常规教学组的75.00%,差异有统计学意义(P<0.05)。结论应用临床路径教学法联合Mini-CEX评估法能够激发学员的学习兴趣,有利于学员理论知识的掌握;并且在提升临床思维、增强独立解决问题能力、提升医患沟通技巧等方面较常规教学法有明显的优势。
Objective To explore the application effect of clinical pathway teaching method combined with mini clinical evaluation exercise(Mini-CEX)in standardized training(referred to as residency training)for neurosurgical resident physicians.Methods A total of 32 postgraduate student of resident training i n the department of neurosurgery of the First Affiliated Hospital of Harbin Medical University from September 2021 to June 2023 were selected as the research subjects.They were randomly divided into the conventional teaching group and the clinical pathway teaching group using a random number table method,with 16 students in each group.The conventional teaching group adopted the combination of conventional teaching method and Mini-CEX evaluation method for teaching,while the clinical pathway teaching group adopted the combination of conventional teaching method and Mini-CEX evaluation method for teaching.At the end of the semester,theoretical knowledge and clinical skills exams,Mini-CEX assessments,and satisfaction surveys were conducted on students,the teaching effectiveness o f the two groups was compared.Results The theoretical knowledge test scores[(52.74±4.28)points]of the clinical pathway teaching group was higher than that of the conventional teaching group[(49.26±3.73)points],and the difference was statistically significant(P<0.05).The scores of communication skills[(7.89±0.94)points],clinical judgment[(7.16±0.73)points],organizational effectiveness[(8.20±0.75)points]and overall performance[(7.71±0.83)points]in Mini-CEX assessment of students in the clinical pathway teaching group were significantly higher than those in the conventional teaching group[(7.23±0.72)p o i n t s,(6.54±0.94)points,(7.45±0.93)points,(7.10±0.81)points],the differences were statistically significant(P<0.05).The total satisfaction of students in the clinical pathway teaching group was 100%,which was higher than 75.00%in the conventional teaching group,and the difference was statistically significant(P<0.05).Conclusion The application of clinical pathway teaching method combined with Mini-CEX assessment method can stimulate students'learning interest and help students to master theoretical knowledge.Compared with conventional teaching method,it has obvious advantages in improving clinical thinking,enhancing independent problem-solving ability,and improving doctor-patient communication skills.
