衰弱对老年类风湿关节炎患者焦虑抑郁、免疫功能和认知功能的影响

Effects of Frailty on Anxiety and Depression,Immune Function and Cognitive Function in Elderly Patients with Rheumatoid Arthritis

摘要 目的:观察衰弱对老年类风湿关节炎(RA)患者焦虑抑郁、免疫功能和认知功能的影响。方法:选取2019年1月至2021年12月我院收治的138例RA患者,根据是否发生衰弱将患者分为衰弱组和非衰弱组。对比衰弱组、非衰弱组的焦虑抑郁评分、免疫功能相关指标和认知功能评分变化情况。结果:138例老年RA患者有91例存在衰弱,47例无衰弱。根据是否衰弱将患者分为衰弱组(n=91)和非衰弱组(n=47)。衰弱组的焦虑自评量表(SAS)、抑郁自评量表(SDS)评分高于非衰弱组(P<0.05)。衰弱组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)低于非衰弱组,CD8^(+)高于非衰弱组(P<0.05)。衰弱组的简易精神状态评估量表(MMSE)评分、蒙特利尔认知评估量表(MoCA)评分低于非衰弱组(P<0.05)。结论:衰弱可以增加老年RA患者的焦虑抑郁程度,还可导致免疫功能和认知功能下降。 Objective:To observe the effects of frailty on anxiety and depression,immune function and cognitive function in elderly patients with rheumatoid arthritis(RA).Methods:138 RA patients who were admitted to our hospital from January 2019 to December 2021 were selected,and patients were divided into frailty group and non-frailty group according to whether frailty occurred.The changes of anxiety and depression scores,immune function related indicators and cognitive function scores in frailty group and non-frailty group were compared.Results:Among 138 elderly RA patients,91 had frailty,and 47 had no frailty.Patients were divided into frailty group(n=91)and non-frailty group(n=47)according to whether they were frail or not.The scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)in frailty group were higher than those in non-frailty group(P<0.05).CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in frailty group were lower than those in non-frailty group,and CD8^(+)was higher than that in non-frailty group(P<0.05).The Mini-Mental State Examination(MMSE)score and Montreal Cognitive Assessment(MoCA)score in frailty group were lower than those in non-frailty group(P<0.05).Conclusion:Frailty can increase the degree of anxiety and depression in elderly RA patients,and can also lead to a decline in immune function and cognitive function.
