摘要
目的 探讨基于患者报告结局的颈肩部功能锻炼在甲状腺癌患者围手术期护理中的应用效果。方法 将该院2022年6月至2023年3月收治的106例行甲状腺癌手术治疗的患者随机分为2组,对照组53例患者接受常规护理干预,观察组53例患者接受早期颈肩部功能锻炼,评估2组患者术后24 h、72 h、1个月及3个月的颈部功能情况、术后1个月生活质量。结果 2组患者术后72 h、1个月、3个月中文改编版颈部疼痛和残疾量表(NPDS)评分比较,差异均有统计学意义(P<0.05)。2组患者术后24 h NPDS评分比较,差异无统计学意义(P>0.05)。2组术后1个月时患者生活质量各维度(心理、感官、疲乏、口腔咽喉、支持、颈部不适)评分比较,差异均有统计学意义(P<0.05);但在声音、头发维度上比较,差异均无统计学意义(P>0.05)。结论 以患者报告结局为导向的颈肩部功能锻炼有助于甲状腺癌患者术后颈肩部不适,提高生活质量。
Objective To explore the effectiveness of neck and shoulder rehabilitation training based on patient-reported outcomes in the perioperative care of patients with thyroid cancer.Methods A total of 106 patients with thyroid cancer surgery treatment randomly divided into two groups,the control group of 53 patients with routine care intervention,observation group 53 patients with early neck shoulder rehabilitation training,evaluate the postoperative neck function in 24 h,72 h,1 month,and 3 months,thyroid specific quality of life 1 month after surgery.Results The scores of the Chinese-adaptated shoulder and Neck Pain and Disability Scale(NPDS) in the two groups were compeared at 72 h,1 month and 3 months after surgery,and the differences were statistically significant(P<0.05),but there was no statistically difference at 24 h after surgery(P>0.05).At 1 month after surgery,the scores of psychological,sensory,fatigue,oral and throat,support,and neck discomfort in the two groups were significant(P<0.05);there was no significant difference in voice and hair dimensions(P>0.05).Conclusion Patient-reported outcome-oriented neck and shoulder rehabilitation training helps with postoperative neck and shoulder discomfort and improve the quality of life in thyroid cancer patients.
