摘要
目的探讨血清同型半胱氨酸(homocysteine,Hcy)与老年帕金森病(Parkinson disease,PD)精神障碍患者精神症状变化关系及预测价值。方法选取2020年6月至2023年6月于台州市第二人民医院诊治的112例老年PD精神障碍患者(PD组)作为研究对象,同期选取55例无精神障碍的老年PD患者(PD无精神障碍组)和56名健康体检者(对照组)作为研究对象,对比三组纳入者的血清Hcy变化。结果PD组患者的Hcy及汉密尔顿焦虑量表(Hamilton anxiety rating scale,HAMA)评分、汉密尔顿抑郁量表(Hamilton depression rating scale,HAMD)评分均高于PD无精神障碍组和对照组;简易精神状态量表(mini mental state examinationscale,MMSE)评分、蒙特利尔认知量表(Montreal cognitive assessment scale,MoCA)评分低于PD无精神障碍组和对照组(P<0.05)。Kendall's tau-b相关性分析显示,血清Hcy与Hoehn-Yahr分级及MMSE、MoCA、HAMA、HAMD评分呈正相关(P<0.05)。受试者操作特征曲线分析显示,Hcy预测老年PD精神障碍患者重度认知功能障碍、焦虑及抑郁的曲线下面积分别为0.848、0.864、0.846。结论血清Hcy在老年PD精神障碍患者中呈异常升高趋势,且其水平随患者精神症状恶化而升高。
Objective To investigate the relationship between serum homocysteine(Hcy)and mental symptoms in elderly patients with Parkinson disease(PD)and its predictive value.Methods A total of 112 elderly patients with PD mental disorder(PD group)who were treated and treated in Taizhou Second People’s Hospital from June 2020 to June 2023 were selected as the study objects,and 55 elderly PD patients without mental disorder(without mental disorder PD group)and 56 healthy physical examination subjects(control group)were selected as the study objects.The serum Hcy changes of the three groups were compared.Results The scores of Hcy,Hamilton anxiety rating scale(HAMA)and Hamilton depression rating scale(HAMD)in PD group were higher than those in without mental disorders PD group and control group,while the scores of mini mental state examinationscale(MMSE)and Montreal cognitive assessment scale(MoCA)were lower than those in without mental disorders PD group and control group(P<0.05).Kendall’s tau-b correlation analysis showed that serum Hcy was positively correlated with Hoehn-Yahr grade,MMSE grade,MoCA grade,HAMA grade and HAMD grade(P<0.05).Receiver operating characteristic curve analysis showed that the area under the curve values of Hcy in predicting major cognitive dysfunction,anxiety and depression in elderly PD patients with mental disorders were 0.848,0.864 and 0.846.Conclusion Serum Hcy showed an abnormal increase trend in elderly patients with PD mental disorder,and its level would increase with the deterioration of patients’mental symptoms.
