自动化眼位训练仪设计及其在翼状胬肉患者中的应用价值

摘要 目的设计一款自动化眼位训练仪,并评价其在翼状胬肉患者中的应用价值。方法前瞻性病例对照研究。选择2021年12月至2023年1月在我院眼科进行手术治疗的翼状胬肉患者作为研究对象,将患者随机分为实验组(n=66)和对照组(n=66),对照组患者采用传统方式进行术前的眼位固视训练,实验组患者术前采用自动化眼位训练仪进行训练,比较两组患者术中眼位情况、术中眼位发生偏移例数、手术配合度、并发症发生率等。结果两组患者在患者头部晃动导致术眼偏离显微镜照明范围次数、术中调整眼位次数、术中调整显微镜次数、指令转向配合错误次数以及术中眼位发生偏移次数等方面差异均无统计学意义(均P>0.05)。但实验组患者自主转眼次数(1.59±1.64)vs.(4.24±1.15)和手术时间(38.73±9.56)min vs.(71.29±17.75)min明显低于对照组(均P<0.05)。实验组患者主动调整眼位次数(2.76±1.39)vs.(1.76±1.28)和术中单次最长保持注视的时间(2.09±0.67)min vs.(1.23±0.72)min明显高于对照组(均P<0.05)。实验组与对照组患者在治愈率和复发率上的差异均无统计学意义(均P>0.05)。实验组患者结膜缝线松脱的发生率明显低于对照组(P<0.05),但在结膜撕裂和角膜擦伤等发生率上的差异均无统计学意义(均P>0.05)。结论术前采用自动化眼位训练仪能减少翼状胬肉的手术时长和并发症,自动化眼位训练仪具有临床应用价值。 Objective To designe an automated eye position trainer and to evaluate application in patients with pterygium.Methods A prospective case-control study.Patients with pterygium who underwent surgery in our hospital from December 2021 to January 2023 were collected and randomly divided into the experimental group(n=66)and control group(n=66).The patients in the control group received preoperative eye position fixation training in the traditional way,and the patients in the experimental group were trained with automatic eye position training instrument before operation.The intraoperative eye position,the number of cases of intraoperative ocular position deviation,the degree of surgical fit and the incidence of complications were compared between the two groups.Results There were no significant differences between the two groups in the number of times the eyes deviated from the illumination range of the microscope caused by head shaking,the times of adjusting the eye position during the operation,the times of adjusting the microscope during the operation,the times of mismatch of command steering and the times of deviation of the eye position during the operation(P>0.05).However,the times of spontaneous eye rotation and the time of operation in the experimental group were signifi⁃cantly lower than those in the control group(1.59±1.64)vs.(4.24±1.15)and(38.73±9.56)min vs.(71.29±17.75)min.The ac⁃tive eye position adjustment times(2.76±1.39)min vs(.1.76±1.28)min and the longest fixation time(2.09±0.67)min vs(.1.23±0.72)min in the experimental group were significantly higher than those in the control group(all P<0.05).There was no significant difference in the cure rate between the experimental group and the control group(all P>0.05),The incidence of conjunctival suture loosening was significantly lower in the experimental group than in the control group(P<0.05),but there were no statistically significant differences in the incidence of conjunctival tears and corneal abrasions(all P>0.05).Conclusions Preoperative automatic eye position fixation training instrument can reduce the operation time,complica⁃tions and recurrence rate of pterygium,and the automatic eye position training instrument has clinical application value.
