摘要
目的:探讨深度镇静下无痛支气管镜术中发生低氧血症的危险因素。方法:回顾性分析2021年3—4月于浙江省肿瘤医院行深度镇静支气管镜检查的293例患者的临床资料。根据术中是否发生低氧血症分为低氧血症组(n=63)与非低氧血症组(n=230),统计两组临床基本资料并进行单因素分析,进而采用多因素logistic回归分析术中发生低氧血症的独立危险因素。结果:低氧血症组体重指数(BMI)≥24 kg/m^(2)、有打鼾史、基线血氧饱和度≤97%、丙泊酚总用量>170 mg的患者占比均高于非低氧血症组,差异有统计学意义(P<0.05)。多因素logistic回归分析表明BMI≥24 kg/m^(2)、有打鼾史、基线血氧饱和度≤97%、丙泊酚总用量>170 mg均为深度镇静下无痛支气管镜术中发生低氧血症的独立危险因素(P<0.05)。结论:BMI≥24 kg/m^(2)、有打鼾史、基线血氧饱和度≤97%、丙泊酚总用量>170 mg是深度镇静下无痛支气管镜术中发生低氧血症的独立危险因素。临床上可通过加强对上述因素的筛查和主动干预,一定程度上减少术中低氧血症的发生。
Objective:To investigate the risk factors of hypoxemia during painless bronchoscopy under deep sedation.Method:The clinical data of 293 patients who underwent painless bronchoscopy under deep sedation in Zhejiang Cancer Hospital from March to April 2021 were retrospectively analyzed.The patients were divided into the hypoxemia group(n=63)and the non-hypoxemia group(n=230)according to whether hypoxemia occurred during the operation.The basic clinical data of the two groups were statistically analyzed and univariate analysis was performed,and then the independent risk factors of hypoxemia during the operation were analyzed by multivariate logistic regression.Result:The proportion of patients with body mass index(BMI)≥24 kg/m^(2),history of snoring,baseline oxygen saturation≤97%,and total dosage of Propofol>170 mg in hypoxemia group were higher than those in non-hypoxemia group,the differences were statistically significant(P<0.05).Multi-factor logistic regression analysis showed that BMI≥24 kg/m^(2),history of snoring,baseline oxygen saturation≤97%,and total doage of Propofol>170 mg were all independent risk factors for hypoxemia during painless bronchoscopy under deep sedation(P<0.05).Conclusion:BMI≥24 kg/m^(2),history of snoring,baseline oxygen saturation≤97%,and total dosage of Propofol>170 mg were independent risk factors for hypoxemia during painless bronchoscopy under deep sedation.The occurrence of hypoxemia can be reduced to some extent by strengthening the screening and active intervention of the above factors in clinic.
