摘要
目的通过测定血清红细胞分布宽度(RDW)、总胆红素(TBIL)及游离三碘甲状腺原氨酸(FT3)水平,分析三者与慢性心力衰竭(CHF)发生、发展的相关性,探讨三者在CHF中的临床意义。方法选取2021年10月至2022年10月就诊于鞍山市中心医院且符合标准的160例慢性心力衰竭患者(心衰组)与141例同期就诊的无严重肝、肾功能不全的非心力衰竭者(对照组)为研究对象,收集其一般临床资料,测得血清红细胞分布宽度变异系数(RDW-CV)、TBIL、FT3水平。心衰组患者按照纽约心脏病协会(NYHA)心功能分级分为Ⅱ级55例、Ⅲ级51例和Ⅳ级54例,统计分析心衰组与对照组间及心衰组组间的上述指标差异性,研究上述指标与CHF患者心功能分级的相关性,探讨RDW、TBIL、FT3在CHF患者中的联合诊断价值。结果RDW-CV、TBIL与心功能分级呈正相关,FT3与心功能分级呈负相关,三者联合预测概率的曲线下面积(AUC)大于单一因素。结论RDW-CV、TBIL与FT3是CHF的预测因素,RDW-CV、TBIL与心功能分级呈正相关,FT3与心功能分级呈负相关,三者联合检测CHF的价值高于单一因素。
Objective To analyze the relationship between serum red blood cell distribution width(RDW),total bilirubin(TBIL),and free triiodothyronine(FT3)levels,and to explore the clinical significance of the three indexes in congestive heart failure(CHF).Methods A total of 160 patients with chronic heart failure(heart failure group)who met the criteria and 141 patients with non-heart failure without severe liver and kidney insufficiency(control group)who were admitted to Anshan Central Hospital from October 2021 to October 2022 were selected as the study subjects.The patients in the heart failure group were divided into gradeⅡ(55 cases),gradeⅢ(51 cases)and gradeⅣ(54 cases)according to the New York Heart Association(NYHA)cardiac function classification,and the differences of the above indexes between the heart failure group and the control group were statistically analyzed,and the correlation between the above indicators and the cardiac function grade of CHF patients was studied,and the combined diagnostic value of RDW,TBIL and FT3 in CHF patients was discussed.Results RDW-CV and TBIL were positively correlated with cardiac function grade,and FT3 was negatively correlated with cardiac function grade,and the area under the curve(AUC)of the combined prediction probability of the three was greater than that of a single factor.Conclusion RDW-CV,TBIL and FT3 are predictors of CHF,RDW-CV and TBIL are positively correlated with cardiac function grade,and FT3 is negatively correlated with cardiac function grade.
