摘要
目的:探讨超声弹性成像(UE)术前评估对肩袖撕裂(RCT)手术患者远期预后的预测价值。方法:根据预后情况不同将106例行关节镜下肩袖修补术的RCT患者分为再撕裂组(n=38)与愈合组(n=68)。比较两组一般临床资料、术前UE检测值[剪切波速度(SWV)、弹性模量]、Constant评分,分析术前SWV、弹性模量与术后Constant评分的相关性,绘制受试者工作特征(ROC)曲线,评估术前SWV、弹性模量单独及联合预测术后再撕裂的效能。结果:再撕裂组术前SWV、弹性模量值及术后Constant评分均小于愈合组(P<0.05);Pearson相关性分析显示,术前SWV、弹性模量与术后1年Constant评分均呈正相关关系(P<0.05);ROC曲线显示,术前SWV敏感度为82.10%,特异度为75.00%;弹性模量敏感度为80.40%,特异度为73.60%;联合预测敏感度为91.70%,特异度为70.30%,术前SWV、弹性模量及联合检测的ROC曲线下面积(AUC)分别为0.828、0.813、0.949。结论:RCT术前通过UE测量SWV及弹性模量可在一定程度上预测远期预后,术前SWV联合弹性模量对术后再撕裂的预测价值高于单一指标。
Objective:To explore the predictive value of preoperative ultrasound elastography(UE)assessment for the long-term prognosis of patients undergoing rotator cuff tear(RCT)surgery.Methods:106 patients who underwent arthroscopic rotator cuff repair were divided into re-tear group(n=38)and healing group(n=68)according to different prognosis.The general clinical data,preoperative UE measurements[shear wave velocity(SWV),elastic modulus],and Constant scores were compared between the two groups.The correlation between preoperative SWV,elastic modulus,and one-year postoperative Constant scores was analyzed.Receiver operating characteristic(ROC)curves were constructed to evaluate the efficacy of preoperative SWV,elastic modulus alone,and in combination in predicting postoperative re-tear.Results:The preoperative SWV and elastic modulus values,as well as the postoperative Constant scores were lower in the re-tear group than in the healing group(P<0.05).Pearson s correlation analysis showed that preoperative SWV and elastic modulus were positively correlated with one-year postoperative Constant scores(P<0.05).The ROC curves demonstrated that the sensitivity/specificity of preoperative SWV,elastic modulus alone,and in combination for predicting postoperative re-tear were 82.10%/75.00%,80.40%/73.60%,and 91.70%/70.30%,respectively,with corresponding AUC s of 0.828,0.813,and 0.949.Conclusion:Preoperative measurement of SWV and elastic modulus through UE can predict the long-term prognosis of RCT to a certain extent,and combined detection has predictive value for postoperative re-tear.
