摘要
目的:探究内镜下肛直线上套扎术与硬化剂注射治疗Ⅱ~Ⅲ度内痔患者的疗效。方法:选取300例Ⅱ~Ⅲ度内痔患者为研究对象,按照治疗方案不同分为套扎组(n=145)与注射组(n=155)。套扎组行内镜下肛直线上套扎术治疗;注射组行内镜下硬化剂注射治疗。对比两组患者围手术期指标、并发症、肛部视觉模拟评分(VAS)与出血评分、肛门功能恢复情况、临床疗效及复发率。结果:套扎组患者术中出血量低于注射组(P<0.05);两组患者手术并发症发生率无统计学差异(P>0.05);术后,两组患者VAS与出血评分及肛管舒张压与静息压均下降,且套扎组低于注射组(P<0.05);套扎组疗效高于注射组(P<0.05);两组患者复发率无统计学意义差异(P>0.05)。结论:内镜下肛直线上套扎术与硬化剂注射均可有效治疗内痔,止血效果相当,套扎术术后远期提拉效果更好,肛门功能恢复情况更好,值得应用推广。
Objective:To explore the efficacy of endoscopic anorectal line ligation and endoscopic injection sclerotherapy in treating patients withⅡ~Ⅲinternal hemorrhoids.Methods:The patients with 300Ⅱ~Ⅲinternal hemorrhoids admitted were retrospectively analyzed.According to different treatment regimens,they were divided into ligation group(n=145,endoscopic anorectal line ligation)and injection group(n=155,endoscopic injection sclerotherapy).The perioperative indicators,complications,anal pain(VAS)score and bleeding score,anal function recovery status,clinical efficacy and recurrence rate were compared between both groups.Results:The intraoperative blood loss in ligation group was less than that in injection group(P<0.05).The incidence rates of surgical complications were not statistically different between the two groups(P>0.05).After surgery,the VAS score,bleeding score,anal diastolic blood pressure and resting pressure in the two groups were decreased,and the indicators in ligation group were lower(P<0.05).The efficacy was higher in ligation group compared to injection group(P<0.05).There was no statistical significance in recurrence rate between the two groups(P>0.05).Conclusion:Endoscopic anorectal line ligation and endoscopic injection sclerotherapy can effectively treat internal hemorrhoids,the hemostatic effect is comparable,the long-term lifting effect and recovery of anal function are better after ligation surgery.It is worth promoting and applying.
