摘要
目的分析超声引导下扩张关节囊治疗冻结肩的有效性及安全性。方法队列研究。抽取2020年6月至2023年3月于宝鸡市中医医院确诊的冻结肩患者282例为研究对象,根据随机数字表法分为对照组与研究组,每组141例。对照组行超声引导下关节腔注射治疗,研究组行超声引导下关节腔注射与关节囊扩张治疗,两组均持续治疗3次。比较两组治疗前后肩关节囊厚度、肩关节功能[肩关节主动活动范围(AROM)评分]、疼痛程度[视觉模拟量表(VAS)评分]。结果治疗前,两组肩关节囊厚度、AROM评分、VAS评分比较差异均未见统计学意义(P均>0.05)。治疗6、12周后,研究组肩关节囊厚度小于对照组,AROM评分高于对照组,VAS评分低于对照组(P<0.05)。结论超声引导下扩张关节囊联合关节腔注射治疗冻结肩者效果良好,可有效改善关节囊厚度,促进肩关节功能恢复,并减轻疼痛感。
Objective To analyze the efficacy and safety of ultrasound-guided capsule distention in the treatment of frozen shoulder.Methods A total of 282 patients diagnosed with frozen shoulder in Baoji Hospital of Traditional Chinese Medicine from June 2020 to March 2023 were selected as research objects of the cohort study.And they were divided into the control group and the study group by random number table method,with 141 cases in each group.The control group received ultrasound-guided articular capsule injection,and the study group was treated by ultrasound-guided articular capsule injection and articular capsule distention.Both groups were continuous treated for 3 times.The shoulder capsule thickness,shoulder function evaluated by active range of motion score(AROM)score,and pain degree assessed by visual analogue scale(VAS)score were compared between the two groups before and after treatment.Results There was no significant difference in shoulder capsule thickness,AROM score and VAS score between the two groups before treatment(all P>0.05).After 6 and 12 weeks of treatment,the study group had thicker shoulder capsule thickness,higher AROM score and lower VAS score compared with the control group(P<0.05).Conclusions Ultrasound-guided capsule distention combined with articular capsule injection is effective in the treatment of frozen shoulder,which can effectively improve the thickness of joint capsule,promote the recovery of shoulder function and relieve pain.
