摘要
目的:探讨老年高血压患者左心室肥厚的影响因素。方法:选取2021年6—12月在上海市浦东新区金杨社区卫生服务中心参与体检的65岁及以上的365例老年高血压患者。以左心室质量指数作为判断标准,将其分为左心室肥厚组(134例)和左心室正常组(231例)。分析两组患者的人口学资料和相关危险因素,采用多因素logistic模型分析左心室肥厚的影响因素。结果:老年高血压患者左心室肥厚发生率为36.7%。与左心室正常组相比,左心室肥厚组年龄更大、女性占比更高、高血压病程更长、体质指数(BMI)更高,差异有统计学意义(P<0.05)。年龄(OR=1.095)、女性(OR=4.718)、高血压病程(OR=1.039)、BMI(OR=1.136)为老年高血压患者左心室肥厚的危险因素。结论:高龄、女性、高血压病程长、肥胖是老年高血压患者左心室肥厚的危险因素。
OBJECTIVE To explore the influencing factors of left ventricular hypertrophy(LVH)in community elderly patients with hypertension.METHODS A total of 365 hypertensive patients over 65 years old who participated in physical examination in Jin⁃yang Community Health Service Center,Pudong New Area,Shanghai from June 2021 to December 2021 were selected and divided into LVH group(n=134)and non-LVH group(n=231)using left ventricular mass index as the criterion for LVH.The demographic data and risk factors of the two groups were analyzed,and the influencing factors of LVH were analyzed by multiple logistic regres⁃sion.RESULTS The incidence of LVH in the elderly patients with hypertension was 36.7%.Compared with non-LVH group,LVH group was older,the proportion of females was higher,the course of hypertension was longer,and the body mass index(BMI)was higher(P<0.05).The age(OR=1.095),female(OR=4.718),the duration of hypertension(OR=1.039)and BMI(OR=1.136)were the risk factors of LVH in the elderly patients with hypertension.CONCLUSION Old age,female,long course of hypertension and obesi⁃ty are the risk factors for LVH in elderly hypertensive patients in community.
