摘要
目的 回顾性评价应用内消连翘丸治疗甲状腺结节的临床疗效。方法 选取2013年—2020年就诊于北京中医医院内分泌科诊断为结节性甲状腺肿的患者729例。暴露组符合纳排标准,应用内消连翘丸共421例;对照组符合纳排标准,未应用内消连翘丸共308例。疗程6个月及以上。观察甲状腺最大结节体积、甲状腺结节性质评分以及各风险指标、症状积分、甲状腺相关抗体。结果 治疗后治疗组甲状腺最大结节体积较治疗前降低,且治疗组均低于对照组(P<0.05)。治疗后治疗组甲状腺结节性质评分较治疗前降低,且治疗组均低于对照组(P<0.05);治疗后治疗组边缘不规则、点状强回声细小钙化灶、结节纵横比>1风险指标较治疗前降低,且治疗组均低于对照组(P<0.05);低回声实性结节、甲状腺被膜受侵状态差异无统计学意义(P>0.05)。治疗后治疗组症状积分均较治疗前降低,且治疗组症状积分均低于对照组(P<0.05)。治疗后治疗组TGAb、TPOAb较治疗前降低,且治疗组均低于对照组(P<0.05)。结论 内消连翘丸在治疗甲状腺结节上可降低甲状腺结节性质评分、缩小结节,改善结节性质及调节甲状腺免疫状态。
Objective To retrospectively evaluate the clinical efficacy of Neixiao Lianqiao Pill(内消连翘丸)in the treatment of thyroid nodules.Methods From 2013 to 2020,729 patients with nodular goiter were selected from the Department of Endocrinology,Beijing Hospital of Traditional Chinese medicine.In the exposed group,421 cases were treated with Neixiao Lianqiao Pill,while in the control group,308 cases were not treated with Neixiao Lianqiao Pill.The course of treatment was 6 months or more.The largest nodule volume,thyroid nodule score,risk factors,symptom scores and thyroid-associated antibodies were measured.Results After treatment,the volume of the largest thyroid nodule in the treatment group was lower than that before treatment,and that in the treatment group was lower than that in the control group(P<0.05).After treatment,the thyroid nodule scores in the treatment group were lower than those before treatment,and the scores in the treatment group were lower than those in the control group(P<0.05).After treatment,the risk indexes of irregular border,small calcification focus and ratio of longitudinal to transverse of nodules in the treatment group were lower than those before treatment,and those in the treatment group were lower than those in the control group(P<0.05).There was no significant difference in hyperechoic solid nodules and invasion of thyroid capsule(P>0.05).After treatment,the symptom integral of the treatment group was lower than that before treatment,and the symptom integral of the treatment group was lower than that of the control group(P<0.05).After treatment,the levels of anti-thyroglobulin antibodies(TGAb)and anti thyroid peroxidase antibody(TPOAb)in the treatment group were lower than those before treatment,and those in the treatment group were lower than those in the control group(P<0.05).Conclusion Neixiao Lianqiao Pill can reduce the thyroid nodule score,reduce the size of the nodules,improve the quality of the nodules and modulate the immune state of the thyroid in the treatment of thyroid nodule.
