摘要
目的探讨慢性鼻窦炎鼻内镜术后嗅觉障碍改善不良危险因素,旨在为后续临床防治工作提供参考。方法回顾性纳入2021年6月至2023年6月于本院行鼻内镜手术治疗慢性鼻窦炎患者共131例,根据术后嗅觉障碍改善效果分为良好组和不良组,采用单因素和多因素法评估行鼻内镜手术治疗慢性鼻窦炎患者术后嗅觉障碍改善不良独立危险因素。结果131例患者术后嗅觉障碍改善不良43例,不良率为32.82%。单因素分析结果显示,有无过敏性疾病史、有无合并鼻息肉、有无长期接受减充血剂治疗及有无术后遵医治疗均可能与行鼻内镜手术治疗慢性鼻窦炎患者术后嗅觉障碍改善效果有关(P<0.05)。多因素分析结果显示,过敏性疾病史、合并鼻息肉、长期接受减充血剂治疗及术后未遵医治疗均是术后嗅觉障碍改善不良独立危险因素(P<0.05)。结论既往过敏疾病史、伴鼻息肉、长期接受减充血剂治疗及术后未遵医治疗的慢性鼻窦炎患者鼻内镜手术后嗅觉障碍改善效果更差。
Objective To investigate the risk factors of adverse improvement for postoperative olfactory dysfunction in patients with chronic sinusitis treated by nasal endoscopic surgery to provide reference for follow-up clinical prevention and treatment.Methods 131 patients with chronic sinusitis treated by nasal endoscopic surgery in our hospital from June 2021 to June 2023 were retrospectively included.They were divided into good group and poor group according to the improvement effect of postoperative olfactory dysfunction.Univariate and multivariate methods were used to evaluate the independent risk factors of poor improvement of olfactory dysfunction after endoscopic sinus surgery.Results There were 43 cases with poor improvement in olfactory dysfunction in all 131 patients with chronic sinusitis treated by nasal endoscopic surgery in our hospital and the poor rate was 32.82%.The results of univariate analysis showed that history of allergic disease,nasal polyps,long-term decongestant treatment and postoperative compliance may be related to the improvement of postoperative olfactory dysfunction in patients with chronic sinusitis treated by nasal endoscopic surgery(P<0.05).Multivariate analysis showed that allergic disease history,nasal polyps,long-term decongestant treatment and non-compliance were independent risk factors for poor improvement of olfactory dysfunction after endoscopic sinus surgery(P<0.05).Conclusion Patients with chronic sinusitis with history of allergic disease,complicated with nasal polyps,long-term decongestant treatment,and no postoperative treatment had worse improvement effect on olfactory disturbance after nasal endoscopic surgery.
