摘要
目的探讨术前精准化血糖管理对妊娠期糖尿病(GDM)患者妊娠结局的影响。方法选取60例行择期剖宫产手术的GDM患者,根据随机数字表法分成两组,各30例。对照组患者予以常规剖宫产手术护理和血糖监测,观察组在对照组基础上予以术前精准化血糖管理。比较两组患者干预前及分娩前的血糖值、新生儿出生后即刻、30 min、60 min血糖值和新生儿不良妊娠结局发生情况。结果观察组分娩前的血糖值均低于对照组,差异有统计学意义(P<0.05)。观察组新生儿出生后即刻、30 min、60 min血糖值均高于对照组,差异有统计学意义(P<0.05)。观察组新生儿不良妊娠结局发生率3.33%低于对照组的26.67%,差异有统计学意义(P<0.05)。结论术前精准化血糖管理可以有效降低GDM患者血糖水平,改善新生儿出生后的血糖水平,降低新生儿低血糖及窒息发生率,提高分娩质量,值得临床推广与应用。
Objective To investigate the effect of preoperative precision blood glucose management on pregnancy outcomes in patients with gestational diabetes mellitus(GDM).Methods 60 GDM patients undergoing elective cesarean section were selected and divided into two groups with 30 cases in each group according to random number table method.Control group was given routine cesarean section operation nursing and blood glucose monitoring,observation group was given preoperative precise blood glucose management on the basis of control group.Blood glucose values before intervention and delivery,blood glucose values immediately after birth,30 min,60 min,and adverse pregnancy outcomes of neonates were compared between the two groups.Results The blood glucose levels in the observation group before delivery were lower than those in the control group,and the difference was statistically significant(P<0.05).Blood glucose levels immediately after birth,30 min and 60 min in observation group were higher than those in control group,and the difference was statistically significant(P<0.05).The incidence of neonatal adverse pregnancy outcome in the observation group was 3.33%lower than that in the control group(26.67%),and the difference was statistically significant(P<0.05).Conclusion Preoperative precise blood glucose management can effectively reduce the blood glucose level of GDM patients,improve the blood glucose level of newborns after birth,reduce the incidence of neonatal hypoglycemia and other complications,and improve the delivery quality.It is worthy of clinical promotion and application,
