重复经颅磁刺激对酒依赖患者认知功能与BDNF水平的影响

Effects of repeated transcranial magnetic stimulation on cognitive function and BDNF level in alcoholdependent patients

摘要 目的 认知功能障碍是酒精依赖(Alcohol dependence,AD)患者主要精神症状表现之一。目前,重复经颅磁刺激(Repetitive transcranial magnetic stimulation,rTMS)已成为各类精神障碍的有效治疗手段,然而其在酒精依赖治疗中的具体机制和作用尚不清楚。方法 本研究纳入59例AD患者,通过随机双盲方法将受试者分为rTMS刺激组和rTMS安慰刺激组。采用stroop色词测验和威斯康星卡片分类测验评价认知功能,通过检测脑源性神经营养因子(BDNF)水平探讨rTMS调节分子机制的作用。结果 相较于rTMS安慰刺激组,rTMS刺激组有效改善AD患者的认知功能。Stroop色词测验中的词正确数、词遗漏数、字色一致正确数和字色无关正确数项目差异有统计学意义(P<0.05)。威斯康星卡片分类测验中的正确应答数、错误应答数、随机性错误和完成分类数项目差异有统计学意义(P<0.05)。AD患者血清BDNF水平在rTMS刺激后差异有统计学意义(P<0.05)。结论 rTMS能有效改善AD患者的认知功能和血清BDNF水平。 Objective Cognitive impairment is one of the main psychiatric symptoms of alcohol dependence(AD)patients.Repetitive transcranial magnetic stimulation(rTMS)has become an effective treatment for various mental disorders,but its specific mechanism and role in the treatment of alcohol dependence remain unclear.Methods 59 AD patients were included in this study,and divided into rTMS stimulation and rTMS placebo stimulation group through a double-blind randomized experiment.The cognitive function was assessed by stroop color word test and wisconsin card sorting test,and the mechanism of action of rTMS was explored by detecting the level of BDNF.Results Compared with rTMS placebo stimulation group,rTMS stimulation group effectively improved the cognitive function of AD patients.In Stroop color word test,there were significant differences in word correct number,word omitted number,word color consistent correct number and word color independent correct number(P<0.05).In Wisconsin card sorting test,the number of correct responses,the number of false responses,the number of random errors and the number of completed sorting were significant differences(P<0.05).The serum BDNF level in AD patients was significant differences(P<0.05).Conclusion rTMS can effectively improve the cognitive function and serum BDNF level in AD patients.
