摘要
目的研究单一服用氯氮平或利培酮的长期住院男性精神分裂症患者催乳素、促甲状腺激素水平的表达及其与骨密度的相关性。方法收集2020年1月至2020年12月在上海市民政第二精神卫生中心住院的男性精神分裂症患者60例,根据服用抗精神病药物种类分为氯氮平组(28例)和利培酮组(32例),由高年资精神科医师收集核对患者的一般和临床资料;采用X线放射吸收法(RA)进行指骨骨密度值测定;抽取空腹静脉血检测血清钙和无机磷、催乳素、促甲状腺激素的含量,对两组结果进行分析比较。结果两组在年龄、吸烟史、病程、血钙、血磷、体重指数、锻炼时间方面差异无统计学意义(P>0.05);利培酮组骨密度T值低于氯氮平组,差异具有统计学意义(P<0.05);利培酮组催乳素水平高于氯氮平组,差异具有统计学意义(P<0.05);催乳素水平与骨密度T值呈负相关(r=-0.494,P<0.05),促甲状腺激素水平与骨密度T值呈正相关(r=0.421,P<0.05)。结论长期住院男性精神分裂症患者应用氯氮平治疗对患者血清催乳素分泌量、骨质疏松发生的影响程度较利培酮小;使用利培酮治疗的患者应定期监测催乳素水平和骨密度状况。
Objective To study the expression of prolactin and thyroid stimulating hormone levels and their correlation with bone mineral density in long-term hospitalized male schizophrenic patients taking clozapine or risperidone alone.Methods A total of 60 male patients with schizophrenia who were hospitalized in Shanghai Civil Affairs Second Mental Health Center from January 2020 to December 2020 were collected.They were divided into clozapine group(28 cases)and risperidone group(32 cases)according to the types of antipsychotic drugs.The general and clinical data of the patients were collected and checked by senior psychiatrists.Radiographic Absorptiometry(RA)was used to measure the bone mineral density of the phalanx.Fasting venous blood was extracted to detect the contents of serum calcium,inorganic phosphorus,prolactin and thyroid stimulating hormone,and the results of the two groups were analyzed and compared.Results There was no significant difference in age,smoking history,course of disease,blood calcium,blood phosphorus,body mass index and exercise time between the two groups(P>0.05).The T value of bone mineral density in risperidone group was lower than that in clozapine group,and the difference was statistically significant(P<0.05).The prolactin level in risperidone group was higher than that in clozapine group,and the difference was statistically significant(P<0.05).Prolactin level was negatively correlated with bone mineral density T value(r=-0.494,P<0.05),and thyroid stimulating hormone level was positively correlated with bone mineral density T value(r=0.421,P<0.05).Conclusion The effect of clozapine on serum prolactin secretion and osteoporosis in male schizophrenic patients with long-term hospitalization is less than that of risperidone.Prolactin levels and bone mineral density should be monitored regularly in patients treated with risperidone.
