摘要
目的分析血清高迁移率族蛋白B1(HMGB1)、组织金属蛋白酶抑制剂-2(TIMP-2)水平与脓毒症急性肾损伤(AKI)预后的相关性。方法选取2021年1月—2022年12月湖北省武汉市第三医院收治的98例脓毒症并AKI患者,对所有患者行肾脏代替疗法(CRRT)治疗,结合患者治疗后28 d预后状况将其分为两组,A组(预后不良,n=28)和B组(预后良好,n=70),对比两组基础资料,分析影响脓毒症并发AKI患者预后不良的主要因素,分析单一、联合检测对脓毒症并发AKI患者预后不良的诊断效能。结果(1)A组HMGB1、TIMP-2水平及机械通气时间水平高于B组,差异具有统计学意义(P<0.05)。(2)经多因素Logistic回归方程分析,HMGB1、TIMP-2、机械通气时间是预测脓毒症并发AKI预后不良的主要因素(P<0.05)。(3)HMGB1、TIMP-2、机械通气时间联合检测灵敏度、特异度、准确度均高于单一检测,联合检测预测脓毒症并发AKI患者预后不良的价值较高(AUC>0.9)。结论HMGB1、TIMP-2、机械通气时间联合检测预测脓毒症并发AKI患者预后不良有一定价值。
Objective To analyze the correlation between serum high mobility group protein B1(HMGB1)and tissue metalloproteinase-2(TIMP-2)levels and prognosis of acute kidney injury(AKI)in sepsis.Methods Ninety-eight patients with sepsis and AKI admitted to the Third Hospital of Wuhan,Hubei Province from January 2021 to December2022 were selected.All patients were treated with renal replacement therapy(CRRT).Combined with the prognosis of the patients 28 days after treatment,they were divided into two groups:Group A(poor prognosis,n=28)and group B(good prognosis,n=70).The basic data of the two groups were compared to analyze the main factors affecting the poor prognosis of patients with sepsis complicated with AKI,and analyze the diagnostic efficacy of single and combined detection of sepsis complicated with AKI.Results(1)The levels of HMGB1,TIMP-2 and mechanical ventilation time in group A were higher than those in group B,and the differences were significant(P<0.05).(2)After multivariate Logistic regression analysis,HMGB1,TIMP-2 and mechanical ventilation time could be the main factors to predict the poor prognosis of sepsis complicated with AKI(P<0.05).(3)The sensitivity,specificity and accuracy of combined detection were higher than that of single detection of HMGB1,TIMP-2 and mechanical ventilation time,and the combined detection of HMGB1,TIMP-2 and mechanical ventilation time had a higher value in predicting the poor prognosis of patients with sepsis complicated with AKI(AUC>0.9).Conclusion The combined detection of HMGB1,TIMP-2 and mechanical ventilation time has a certain value in predicting the poor prognosis of patients with sepsis and AKI.
