摘要
目的:运用数据挖掘技术分析电针治疗原发性失眠的文献,总结电针治疗原发性失眠文献的杂志及地区分布、穴位、诊断标准、观察指标及电针的参数规律。方法:采用计算机检索中国知网(CNKI)、万方(WF)、维普全文数据库(VIP)、中国生物医学文献服务系统(SinoMed)、Pubmed与Web of Science(SCI)数据库中电针治疗原发性失眠的文献。使用EndNote进行文献重复处理,使用Microsoft Excel 2019进行文献的杂志及地区情况、腧穴使用频次、诊断标准、观察指标及电针的参数分析,使用SPSS 21.0和clementine 12.0数据分析软件进行聚类和关联性分析。结果:共纳入文献92篇。所纳入文献中,杂志分布较分散,作者所在地区以广东省居多。共涉及70个体穴,总频次713次;涉及8个耳穴,总频次15次。神门、三阴交、百会和印堂使用频次较多,百会、四神聪、安眠、三阴交、神门和神庭为常用的电针连接穴位;穴位以膀胱经穴位为主,主要分布在下肢和头面,五输穴的使用频次最高;神门、百会和三阴交为常用的主穴,常用的穴组为神门与安眠和三阴交。最常用的诊断标准为《中国精神障碍分类与诊断标准第3版(CCMD-3)》,最常用的量表为匹兹堡睡眠质量指数(PSQI),常用的电针波形为连续波2 Hz。结论:电针治疗原发性失眠的穴位以局部取穴为主,注重上下配穴;诊断标准以国内的为主,观察指标以主观量表为主;低频连续波为治疗失眠的常用电针波形。
Objective:To analyze literature on electro-acupuncture(EA)treatment for primary insomnia by using data mining techniques,and to summarize the journal and regional situation,acupoint patterns,diagnostic criteria,observation indicators,and parameter patterns of EA in treatment of the disease.Methods:The literature on EA for primary insomnia were retrieved from the databases of CNKI,Wanfang,VIP,SinoMed,Pubmed and Web of Science.EndNote was used for literature duplication processing,Microsoft Excel 2019 was utilized to analyze the journal and regional situation,acupoint frequency,diagnostic criteria,observation indicators,and parameters of EA in the literature,and SPSS 21.0 and Clementine 12.0 data analysis software were applied for clustering and correlation analysis.Results:92 pieces of literature were included,among which,the distribution of journals was relatively scattered,with the majority of authors located in Guangdong province.A total of 70 body acupoints were involved with a total frequency of 713 times of application,and 8 ear acupoints were included with a total frequency of 15 times of application.Shenmen(HT7),Sanyinjiao(SP6),Baihui(DU20)and Yintang(DU29)were used more frequently.DU20,Sishencong(EX-HN1),Anmian(EX-HN54),SP6,HT7 and Shenting(DU24)were commonly intervened with EA.The acupoints chosen were mainly from Bladder meridian,locating in the lower limbs and head,with the highest frequency of application of five-Shu acupoints.HT7,DU20 and SP6 were commonly used main acupoints,and the commonly used acupoint groups was HT7-EX-HN54-SP6.The most commonly used diagnostic criteria were the Chinese Classification and Diagnostic Criteria for Mental Disorders(3rd Edition),the most commonly used scale was the Pittsburgh Sleep Quality Index(PSQI),and the commonly used EA waveform was a continuous wave of 2 Hz.Conclusion:The acupoints of EA for primary insomnia mainly focuses on local acupoints and emphasizes the combination of upper and lower acupoints.The diagnostic criteria are mainly based on domestic standards,and the observation indicators mainly rely on subjective scales.Low frequency continuous wave is a commonly used EA waveform for treating insomnia.
