母乳嗅觉、味觉刺激联合非营养性吸吮对早产儿喂养耐受性及发育情况的影响

Effect of Olfactory and Gustatory Stimulation of Breast Milk Combined with Non-nutritive Sucking on Feeding Tolerance and Development in Premature Infants

摘要 目的:探讨母乳嗅觉、味觉刺激联合非营养性吸吮在早产儿中的应用效果。方法:选取2021年9月—2023年9月莆田市第一医院收治的86例早产儿作为研究对象,按随机数表法分为两组,各43例。对照组采取常规干预,观察组采取母乳嗅觉、味觉刺激联合非营养性吸吮干预。对比两组康复相关指标、喂养耐受情况、吮吸能力及发育情况。结果:观察组每次残留奶量低于对照组,每日排便次数多于对照组,胎粪排尽时间、达全肠道喂养时间、胃管拔除时间、喂养过渡时间、达出生体重时间短于对照组,差异有统计学意义(P<0.05)。观察组喂养不耐受总发生率为4.65%,低于对照组的18.60%,差异有统计学意义(P<0.05)。干预后,观察组喂养效率、吮吸速率高于对照组,差异有统计学意义(P<0.05)。观察组出生14天体重增长、头围增长、身长增长均高于对照组,差异有统计学意义(P<0.05)。结论:母乳嗅觉、味觉刺激联合非营养性吸吮可以缩短早产儿经口喂养时间,改善吮吸能力,减少喂养不耐受,促进身体发育。 Objective:To investigate the effects of olfactory and gustatory stimulation of breast milk combined with non-nutritive sucking in premature infants.Method:A total of 86 premature infants admitted to the First Hospital of Putian City from September 2021 to September 2023 were selected as the study objects and divided into two groups according to random number table method,with 43 cases in each group.The control group received routine intervention,and the observation group received olfactory and gustatory stimulation of breast milk combined with non-nutritive sucking intervention.The indexes related to rehabilitation,feeding tolerance,sucking ability and development of the two groups were compared.Result:The amount of residual milk per time in the observation group was lower than that in the control group,the number of daily defecation times was higher than that in the control group,and the time of meconium depletion,whole intestinal feeding time,gastric tube removal time,feeding transition time and birth weight time were shorter than those in the control group,the differences were statistically significant(P<0.05).The total incidence of feeding intolerance in the observation group was 4.65%,which was lower than 18.60%in the control group,and the difference was statistically significant(P<0.05).After the intervention,the feeding efficiency and sucking rate of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The weight growth,head circumference growth and length growth of 14 d after birth of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion:Olfactory and gustatory stimulation of breast milk combined with non-nutritive sucking can shorten the oral feeding time of premature infants,improve sucking ability,reduce feeding intolerance,and promote physical development.
