摘要
目的:探讨甲状腺功能亢进(甲亢)患者甲状腺激素水平与胰岛素抵抗、糖代谢功能紊乱的相关性。方法:选取抚州市第一人民医院2022年10月—2023年11月收治的42例甲亢患者,设为研究组,另选取医院同期行健康体检的42例健康人,设为对照组;检测并对比两组甲状腺激素水平[促甲状腺激素(TSH)、游离甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))]、胰岛素抵抗指数(HOMA-IR)、血糖水平[空腹血糖(FPG)、糖化血红蛋白(HbA1c)],经Pearson相关性检验分析甲状腺功能亢进患者甲状腺激素水平与胰岛素抵抗、糖代谢功能紊乱的关系。结果:研究组TSH水平较对照组低,FT_(3)、FT_(4)、HOMA-IR、FPG、HbA1c水平均较对照组高,差异均有统计学意义(P<0.05);经Pearson相关性检验分析显示,甲亢患者TSH水平与HOMA-IR、FPG、HbA1c水平均呈负相关(r<0,P<0.05);FT_(3)、FT_(4)水平与HOMA-IR、FPG、HbA1c水平均呈正相关(r>0,P<0.05)。结论:甲亢患者甲状腺激素水平与胰岛素抵抗、糖代谢功能紊乱有关,随着甲状腺激素水平的失调,胰岛素抵抗、糖代谢功能紊乱加重。
Objective:To explore the correlation between thyroid hormone levels and insulin resistance,glucose metabolism disorders in patients with hyperthyroidism.Method:A total of 42 patients with hyperthyroidism admitted to First People's Hospital of Fuzhou from October 2022 to November 2023 were selected as the study group,and 42 healthy individuals who underwent health examinations at the same time were selected as the control group.The levels of thyroid hormones[thyroid stimulating hormone(TSH),free triiodothyronine(FT_(3)),free thyroxine(FT_(4))],insulin resistance index(HOMA-IR),blood glucose levels[fasting blood glucose(FPG),glycated hemoglobin(HbA1c)]of two groups were detected and compared,and the relationship between thyroid hormone levels and insulin resistance and glucose metabolism disorders in patients with hyperthyroidism through Pearson correlation test was analyzed.Result:The TSH level in the study group was lower than that in the control group,while the levels of FT_(3),FT_(4),HOMA-IR,FPG and HbA1c were higher than those in the control group,with statistical significance(P<0.05).According to Pearson correlation analysis,TSH level in hyperthyroidism patients was negatively correlated with HOMA-IR,FPG and HbA1c levels(r<0,P<0.05);the levels of FT_(3)and FT_(4)were positively correlated with HOMA-IR,FPG and HbA1c levels(r>0,P<0.05).Conclusion:Thyroid hormone levels in patients with hyperthyroidism are related to insulin resistance and glucose metabolism dysfunction.With the imbalance of thyroid hormone levels,insulin resistance and glucose metabolism dysfunction worsen.
