摘要
发育性髋关节发育不良(developmental dysplasiaof the hip,DDH)是最为常见的骨关节畸形之一,它是指婴儿出生时就存在或者生后继续发育表现出来的一系列髋关节异常的总称:包括髋臼发育不良的稳定髋关节、髋关节半脱位、髋关节完全脱位但可以复位、完全脱位且不能复位[1],奥地利小儿骨科教授Reinhard Graf开创了发育性髋关节发育不良超声检查的静态方法,具有可重复性、规范化、标准化及参考指标客观等优点,因此在全世界被广泛采用。Graf法超声检查的应用使DDH的检查时间窗提前,可以在更早发现此病,从而进行更早期干预,使患者恢复健康,不留后遗症。但由于对Graf法认识不透彻,出现了一些“改良Graf法”,影响诊断准确性及检查可重复性,2013年Graf更新了最新的检查质控要求及操作诀窍和技巧,同时归纳了常见错误原因[2]。本文旨在对Graf法婴儿髋关节超声检查规范解读进行综述。
Developmental Dysplasia of the Hip(DDH)is one of the most common bone and joint malformations,which refers to a series of hip abnormalities that exist at birth or continue to develop after birth,including stable hip joints with acetabular dys‐plasia,hip suxuxation,complete dislocation of hip joints that can be reduced,and complete dislocation that cannot be reduced.Reinhard Graf,a pediatric orthopaedic professor from Austria,pioneered a static ultrasound method for developmental hip dys‐plasia,which has the advantages of repeatability,normalization,standardization and objective reference indicators.So,it is widely adopted around the world.The application of Graf ultrasound can advance the time window for the examination of develop‐mental hip dysplasia,so that doctors can detect the disease earlier and,intervene earlier,while,patients can recover their health without sequelae.However,due to the lack of thorough understanding of Graf method in China,some"improved Graf method"has appeared,which affects the diagnostic accuracy and inspection repeatability.In 2013,Graf updated the latest in‐spection quality control requirements,operation know-how and skills,and summarized the causes of common errors.The pur‐pose of this paper is to interpret the standard of infant hip ultrasonography with Graf method.
