摘要
目的:探讨椎基底动脉延长扩张症(VBD)患者中基底动脉(BA)液体衰减反转恢复(FLAIR)序列血管高信号征(FVH)与病症程度和临床症状的相关性。方法:回顾搜集2018年8月-2023年4月在南京医科大学附属淮安第一医院经头颅MRI及时间飞跃法(TOF)-磁共振血管成像(MRA)确诊为VBD患者67例,按照是否出现VBD相关症状(缺血、脑干及神经压迫等)分为症状组(20例)和无症状组(47例),比较2组患者的临床及影像学资料,分析BA的FVH分级与BA病症程度及临床症状的相关性。结果:症状组患者BA管径、FVH发生率显著高于无症状组(P=0.004,P=0.000),症状组中高分级FVH (2、3级)患者比例明显大于无症状组(χ^(2)=11.501,P=0.001)。VBD患者FVH分级与BA的管径、长度及高度分级呈正相关(r=0.563,P=0.000;r=0.482,P=0.000;r=0.512,P=0.000),FVH高级别组(2、3级)BA长度及宽度显著大于低级别组(0、1级)(P=0.000,P=0.000),分叉高度高于低级别组(P=0.012)。结论:VBD患者FVH发生率高,且FVH分级越高,患者出现症状的可能性越大,椎基底动脉病变更严重;FVH形成机制可能与扩张延长的血管中的缓慢血流及不对称扰流有关。
Purpose:To study the correlation between the vascular hyperintensity(FVH)of basilar artery(BA)in patients with vertebrobasilar dolichoectasia(VBD)and the severity of BA disease and clinical symptoms.Methods:Sixty-seven patients with VBD diagnosed by MRI and time-of-flight MR angiography(TOF-MRA),who were admitted in our hospital from August 2018 to April 2023 were included in the study.They were classified into symptomatic group(20 cases)and non=symptomatic group(47 cases)according to the clinical manifestations(ischemia,the compression of brainstem and nerve,etc.).The clinical and image characteristics of the both groups were compared,and the relationship of the grade of FVH and the severity of BA disease as well as clinincal symptoms were analyzed.Results:Of the 67 VBD patients,the occurrence rate of FVH(P=0.000)and the diameter of BA(P=0.004)in symptomatic group were significantly higher than that in non=symptomatic group.In symptomatic group,the ratio of higher FVH grade(2 or 3)was significantly higher than that in non=symptomatic group(χ^(2)=11.501,P=0.001).The grade of FVH had positive correlation with the diameter(r=0.563,P=0.000),length(r=0.482,P=0.000)and the height grade of BA(r=0.512.P=0.000),respectively.The length(P=0.000),diameter(P=0.000)and height grade of BA(P=0.012)in higher FVH grade(2 or 3)group were significantly greater than that in lower FVH grade(O or 1)group.Conclusion:The occurrence of FVH in VBD patients is high,and the higher the grade of FVH is,the more likely the clinical symptoms occur.FVH may correlate with the slow arterial blood flow and turbulent flow in dilated and extended BA lumen.
