摘要
目的对替加环素致低血糖不良反应的文献报道进行总结和分析,为其临床安全应用提供参考。方法计算机检索PubMed、EMBASE、CNKI和万方数据库,检索时限为各数据库建库起至2023年4月,纳入替加环素致低血糖症的中英文病例报道,两位研究者独立进行文献筛选、数据整理及统计。结果共检索到相关文献24篇,经筛选后纳入中文5篇,英文5篇。所报道的病例中,男性占比70%,年龄中位数为71岁。不良反应的表现有心慌、出汗、手抖,以及伴发头晕、恶心、呕吐等。血糖检测结果最低值为1.1~1.5 mmol/L。50%的病人为严重不良反应。不良反应的发生时间最短的为用药后35 h,最长为用药后第8天。采取的措施一般为停药,予以葡萄糖静脉滴注或静推,病人经及时处理后血糖都能恢复正常。结论临床应用替加环素的过程中应警惕低血糖不良反应,及时发现和处置。
Objective To summarize and analyze the literature on tigecycline-induced adverse reactions to hypoglycemia and to provide a reference for its safe clinical application.Methods Computerized searches of the PubMed,EMBASE,CNKI,and Wanfang databases were performed,with a search time limit of April 2023 from the establishment of each database.Both Chinese and English case reports of tigecycline-induced hypoglycemia were included.Two researchers independently performed the literature screening,data organization and statistical analyses.Results After screening,24 related papers were identified,5 in Chinese and 5 in English.Among the reported patients,70%were male,with a median age of 71 years.The adverse reactions were characterized by panic,sweating,hand tremor,and concomitant dizziness,nausea,and vomiting.The lowest blood glucose test results ranged from 1.1 to 1.5 mmol/L.Serious adverse reactions were noted in 50%of patients.The shortest time of occurrence of adverse reactions was 35 h after the drug was administered,and the longest time was the 8th day after the drug was administered.Generally,the drugs are stopped,glucose is administered intravenously or injected intravenously,and the blood glucose of the patients returns to normal after timely treatment.Conclu⁃sion During the clinical application of tigecycline,adverse reactions to hypoglycemia should be alerted,detected and treated in a timely manner.
