摘要
目的全面了解偏远地区少数民族患者使用互联网预约挂号的真实情况,为完善“互联网+医疗”预约平台机制、切实惠及偏远地区少数民族群体提出针对性的对策与建议。方法采用问卷调查法,于2022年6月-9月,走访云南省4个少数民族偏远地区,对816例少数民族患者和702例汉族患者使用互联网预约挂号的情况进行调查。质性研究:采用半结构式访谈法。于2022年9月采用目的抽样法选取云南省偏远地区25例少数民族患者进行半结构深度访谈,了解影响偏远地区的少数民族患者互联网挂号行为与需求期望的相关补充因素。结果偏远地区的少数民族患者(62.62%)对于互联网预约挂号系统的知晓率远低于汉族患者(70.51%)群体,差异有统计学意义(P<0.05);偏远地区的少数民族患者[(17.38±2.15)分]对于预约系统整体帮扶需求高于汉族患者[(16.23±3.24)分],差异有统计学意义(P<0.05)。质性研究提炼出4个主题,分别为宣传推广不足、被动完成预约、传统固有观念、预约系统期望。结论医疗卫生服务机构需进一步完善智慧医疗预约诊疗体系,加强民族地区“互联网+医疗”平台的建设,提高宣传力度,努力突破数字化信息壁垒,实现智慧医疗适域化、适龄化发展。
Objective To fully understand the real situation of minority patients using Internet appointment registration in remote areas,and to put forward targeted countermeasures and suggestions for improving the“Internet+medical”appointment platform mechanism and effectively benefiting minority groups in remote areas.Methods A questionnaire survey was conducted to investigate the use of Internet appointment registration in 816 minority patients and 702 Han patients in four remote areas of ethnic minorities in Yunnan Province from June to September 2022.Qualitative research:Semi-structured interview was used.In September 2022,a purposeful sampling method was used to select 25 minority patients in remote areas of Yunnan Province for semi-structured in-depth interviews to understand the relevant supplementary factors affecting the Internet registration behavior and demand expectations of minority patients in remote areas.Results The awareness rate of Internet appointment registration system in minority patients(62.62%)in remote areas was much lower than that in Han patients(70.51%),and the difference was statistically significant(P<0.05).The minority patients in remote areas[(17.38±2.15)scores]had higher overall support needs for the appointment system than the Han patients[(16.23±3.24)scores],and the difference was statistically significant(P<0.05).Qualitative research extracted four themes,namely,insufficient publicity and promotion,passive completion of appointments,traditional inherent concepts,and appointment system expectations.Conclusion Medical and health service institutions need to further improve the intelligent medical appointment diagnosis and treatment system,strengthen the construction of"Internet+medical"platform in ethnic minority areas,improve publicity,strive to break through digital information barriers,and realize the localization and age-appropriate development of intelligent medical treatment.
